Zusammenfassung

Kindliche Fußdeformitäten gehören zu den häufigsten Krankheitsbildern in der Kinderorthopädie und dominieren die tägliche Praxis. Kongenitale strukturelle Fehlstellungen müssen von den flexiblen Deformitäten differenziert, sekundäre Fehlstellungen im Rahmen ihrer Grunderkrankung eingeschätzt und behandelt werden. In den letzten Jahren setzte sich für die Klumpfußtherapie ein sehr konservativer Ansatz durch, der ähnlich auch für den angeborenen Plattfuß Verwendung findet. Bei sekundären, meist neuromuskulären Fußfehlstellungen wird der funktionelle Aspekt in der Behandlung stark betont. Die Erhaltung flexibler Füße steht über der Korrektur durch Versteifungsoperationen. Der vorliegende Beitrag beschäftigt sich mit der aktuellen Therapie der bedeutendsten kongenitalen und erworbenen Fußfehlstellungen im Kindesalter.

Abstract

Pediatric foot deformities are known to be the most frequent problem in pediatric orthopaedics. Congenital structural deformities need to be differentiated from flexible and secondary acquired foot anomalies often associated with neuromuscular diseases. In recent years clubfoot treatment has changed to a very conservative approach, which is similarly applied for congenital vertical talus treatment. Secondary foot deformities are treated according to the principles of the underlying neurological condition and therapy depends on functional aspects. The preservation of flexible feet is given priority over stiffening surgery. The present article discusses current treatment strategies for the most important congenital and acquired foot deformities in childhood.
