Zusammenfassung
Je nach Entwicklungsstadium unterscheidet sich das Kind in einer für dieses Stadium spezifischen Weise vom Erwachsenen. In der sehr frühen Phase der Entwicklung weichen die Pharmakokinetik und -dynamik eines Arzneimittels aufgrund der ausgeprägten Unreife vieler Organfunktionen am ausgeprägtesten von der Pharmakologie des Erwachsenen ab. So muss dem Früh- und Neugeborenen meist eine deutlich niedrigere Arzneimitteldosis als aufgrund des Körpergewichts angenommen verabreicht werden. In den weiteren Stadien verläuft die Entwicklung keineswegs linear, so sind z. B. aufgrund der hohen Ganzkörper-Clearance bei Säuglingen und Kleinkindern erheblich höhere relative Arzneimitteldosierungen erforderlich. Nach Inkrafttreten der europäischen Kinderarzneimittelverordnung ist zu hoffen, dass die pharmazeutische Forschung sich der Kinderarzneimittelproblematik wesentlich umfassender annimmt. Es ist medizinisch und ethisch nicht mehr vertretbar, dass Kindern der ihnen laut Arzneimittelgesetz zugesicherte pharmakotherapeutische Zugang verwehrt bleibt. Auf dem Weg zu einer besseren und sichereren Arzneimitteltherapie für Kinder sind ein größeres Problembewusstsein und deutlich mehr Kompetenz in der klinischen Pharmakologie der Pädiatrie bei allen Beteiligten erforderlich.
Abstract
Each stage of childhood development implies specific differences from adults. In the very early stage of development, the extreme immaturity of many organ functions leads to the most pronounced differences in pharmacokinetics and pharmacodynamics compared with adults. This has a very practical impact because a drug for a preterm or term newborn very often must be administered in a much lower dosage than one would assume on the basis of body weight alone. In subsequent developmental stages, the overall development by no means occurs linearly. The total body clearance of an infant or toddler, for example, is much higher than in older individuals, so a relatively much higher dose of a medicine is needed to achieve an effective drug level in the body. After the European Regulation on Medicinal Products for Paediatric Use takes effect, one can hope that all problems concerning drug treatment in children will be much more appreciated by people working in drug research and development. It is no longer acceptable for children to not have pharmacotherapeutic access as guaranteed by law. On the way to better and safer medicines for children, all stakeholders must be more aware of these problems and more competent in clinical pharmacology than has been the case in the past.
