Zusammenfassung
Kinder mit lebenslimitierenden — sowohl malignen als auch nichtmalignen — Erkrankungen sterben nach Elterneinschätzung in den meisten Fällen unter Schmerzen. Tatsächlich schränken schwere Schmerzen in der Lebensendphase die Lebensqualität massiv ein. Fast immer lassen sich auch starke Schmerzen durch eine rationale Therapie beherrschen. Unabdingbare Voraussetzung dafür sind eine ausführliche Schmerzanamnese und die kontinuierliche Schmerzmessung und -dokumentation. Die medikamentöse Schmerzbehandlung erfolgt nach dem WHO-Stufenschema — meist mit starken Opioiden. Umfassend wird der Therapieansatz durch adjuvante Medikamente, invasive schmerztherapeutische Verfahren und nichtmedikamentöse Therapiemöglichkeiten.
Abstract
According to their parents, in most cases children suffering life-limiting diseases—malignant as well as non-malignant—die while under pain. Indeed, at the end-of-life phase pain frequently has a severe impact on quality of life. However, effective pain control, even for the severest pain, almost always exists. The prerequisites for rational pain control are a comprehensive pain history, continuous pain measurement, and meticulous documentation of pain control. Medical pain control should be guided by the World Health Organisation step approach. This often uses strong opioids. In addition, a comprehensive approach uses adjuvants, invasive procedures for pain control, and non-medical treatment.
