Asystole in breath-holding spells

Zusammenfassung

Wir berichten über 2 Kinder, bei denen rezidivierende Anfälle von Bewusstlosigkeit unter dem Bild eines Affektkrampfes auftraten. Bei beiden konnte eine prolongierte Asystolie dokumentiert werden. Wir diskutieren unterschiedliche Formen und Subgruppen von so genannten Affektkrämpfen und die Verbindung zu dem Symptomenkomplex der neurokardiogenen Synkopen. Bestimmte Untergruppen entsprechen nicht dem klassischen Bild der Affektkrämpfe und sollten besser als frühe Manifestation einer neurokardiogenen Synkope klassifiziert werden. Im Gegensatz zu den so genannten Affektkrämpfen hat dies auch Auswirkungen hinsichtlich einer evtl. notwendigen und auch möglichen Therapie.

Abstract

Two cases of children with breath-holding spells and documented prolonged asystole are reported. We discuss the different clinical and pathophysiological subgroups of this entity and the possible connection to neurocardiogenic syncope. For a certain defined subgroup of breath-holding spells, its classification as an early manifestation of neurocardiogenic or neurally mediated syncope would be more correct. This would have implications for further diagnostic as well as therapeutic procedures.
