Zusammenfassung
Hintergrund
Zu den 3 therapeutischen Säulen bei Patienten mit allergischer Rhinitis (AR) zählt neben Karenzverfahren und medikamentöser Therapie die Hyposensibilisierungstherapie oder spezifische Immuntherapie (SIT). Die Effektivität der SIT ist abhängig vom spezifischen Allergen, der Qualität und kumulativer Gesamtdosis der verabreichten Allergenextrakte und dem Applikationsschema. Bei der Cluster-Therapie werden während der Dosissteigerung 2–3 Injektionen je Behandlungstag verabreicht, wodurch das rasche Erreichen der Maximaldosis gewährleistet wird.
Patienten
Wir untersuchten in dieser Studie 79 erwachsene Patienten (18–59 Jahre, Altersdurchschnitt: 27,4±7,2 Jahre) mit einer Hausstaubmilbenallergie hinsichtlich auftretender Nebenwirkungen des Cluster-Therapieschemas in der Dosissteigerungsphase.
Ergebnisse
Es traten 129 übersteigerte Lokalreaktionen (≥ Grad 1) auf, entsprechend 10,2% aller Injektionen (69% Grad-1-, 20% Grad-2-, 11% Grad-3- und keine Grad-4-Reaktionen). Systemische Reaktionen gab es 40-mal, entsprechend 3,2% aller Injektionen, hiervon waren 37 (92%) Stadium-1- und 3 (8%) Stadium-2-Reaktionen. Systemische Reaktionen der Stadien 3 und 4 wurden nicht beobachtet.
Schlussfolgerung
Unter Sicherheitsaspekten ist die Cluster-SIT mit Depotallergenen eine interessante Alternative zu herkömmlichen Therapieschemata für die SIT.
Abstract
Background
Apart from allergen avoidance and pharmacotreatment, specific immunotherapy (SIT) is the most important therapeutic option offered to allergic patients. Clinical efficacy of SIT depends on the specific allergen extract used, the quality and total cumulative amount of allergen applied and the treatment schedule of the therapy. In cluster SIT, 2–3 injections per treatment day are applied resulting in an accelerated achievement of the maintenance dose.
Patients and methods
In this study 79 mite-sensitized adult allergic patients aged 18 to 59 years (mean 27.4±7.2 years) were investigated in terms of side-effects of cluster SIT during induction treatment.
Results
A total of 129 local hypersensitivity reactions (>grade 1) occurred or 10.2% of all injections (69% grade 1, 20% grade 2, 11% grade 3 and no grade 4 reactions). The total number of systemic reactions was 40 or 3.2% of all injections with 92% grade 1 and 8% grade 2 reactions. Grade 3 or 4 reactions did not occur.
Conclusion
Cluster SIT with depot allergens represents an interesting alternative option to conventional dosage schedules of SIT.
