Photodynamische Therapie kutaner T-Zell-Lymphome in besonderer Lokalisation

Zusammenfassung

Die Behandlung der heterogenen Gruppe primär kutaner B- und T-Zell-Lymphome erfolgt stadienadaptiert, aber insbesondere bei den seltenen Formen bisher nicht standardisiert. Wir berichten über 2 Patienten mit primär kutanem T-Zell-Lymphom (CTCL), die neben etablierten Therapieverfahren einer topischen photodynamischen Therapie (PDT) in besonderer und für die klassischen Therapieverfahren nur schwer zugänglicher Lokalisation (Ohr, Augenbraue, Fußkante) zugeführt wurden. Bei Patient 1 handelte es sich um die seltene Variante eines mittelgroßzellig pleomorphen CD8-positiven, CD30-negativen, primär kutanen CTCL, dessen Primärmanifestation am Ohr darüber hinaus eine Rarität darstellt. Nachdem es unter Therapie mit PUVA, Interferon-α und Retinoid zum lokalen progredienten Tumorwachstum gekommen war, erhielt der Patient eine topische PDT am Ohr. Hierunter histologisch gesicherte partielle Remission (PR). Bei erneuter lokaler Krankheitsprogredienz (PD) wurde eine Radiotherapie angeschlossen, seitdem 22 Monate komplette Remission (CR). Bei Patient 2 handelte es sich um das klassische Bild einer Mycosis fungoides im initialen Tumorstadium. Dieser Patient erhielt ergänzend zu PUVA-Therapie und Interferon-α eine topische photodynamische Therapie an den Lokalisationen Augenbraue und Fußkante. Histologisch kontrolliert wurde hier eine komplette Remission (CR) erzielt. Die PDT stellt in der palliativen Behandlung kutaner T-Zell-Lymphome insbesondere in Problemlokalisationen ein neues experimentelles Verfahren dar, das gewebsschonend und nebenwirkungsarm ist, beliebig oft wiederholt werden kann und zu einem guten kosmetischen Ergebnis führt.

Summary

Therapy of the heterogenous group of primary cutaneous B- and T-cell lymphomas is based upon type of lymphoma and its staging. Treatment, especially for rare forms, has not yet been standardised. Two patients with uncommon primary cutaneous T-cell lymphomas received both traditional therapy and topical photodynamic therapy (PDT) in areas that are difficult to reach with classic methods (ear, eyebrow, side of foot). The first patient had a rare type of medium/large cell pleomorphic, CD8-positive, CD30-negative, primary cutaneous T-cell lymphoma with primary involvement of the ear. As the tumor progressed during PUVA, interferon α and retinoid therapy, topical PDT was employed. A histologically confirmed partial remission was obtained. As the disease further evolved the patient received radiotherapy and has now been in complete remission for 10 months. The other patient preserted with the classical picture of mycosis fungoides in initial tumour stage. This patient received in addition to PUVA therapy and interferon α, topical PDT to the eyebrow and side of the foot. A complete remission was achieved and histologically confirmed. In the palliative treatment of cutaneous T-cell lymphoma, PDT is a new experimental method, especially for problem locations. It is tissue sparing, has few side effects, can be repeated as often as necessary and achieves good cosmetic results.
