Zusammenfassung
Die Knötchen der Iris bei Neurofibromatose I haben sich zu einem wichtigen diagnostischen Kriterium in der Differentialdiagnose der Phakomatosen entwickelt. Diese Knötchen wurden in ihrem klinischen Erscheinungsbild und ihrer Bedeutung erstmals von Karl Lisch, München, 1937 erkannt. Diagnose und Differentialdiagnose der Lisch-Knötchen werden illustriert. Auf die Bedeutung der Irisknötchen für die genetische Beratung der Patienten und ihre Familienangehörigen wird, besonders im Hinblick auf monosymptomatische Verläufe, eingegangen. Histologisch zeichnen sich Lisch-Knötchen durch Aggregation von länglichen bis rundlichen Zellen aus, die der Iris kuppelförmig aufsitzen. Immunhistochemisch weisen diese Zellen eine positive Markierung mit einem Antikörper gegen Vimentin und das S-100-Protein auf und belegen damit eine neuroektodermale Differenzierung. Somit können die Lisch-Knötchen als eine direkte Manifestationsform der neuroektodermalen Störung bei Neurofibromatose I angesehen werden.
Summary
Iris nodules in neurofibromatosis I have become an important tool in the differential diagnosis of phakomatoses. The clinical appearance and importance of these nodules first recognized by Karl Lisch in Munich in 1937. The diagnosis and differential diagnosis of Lisch nodules are illustrated. The importance of iris nodules in genetic counselling of patients and their relatives is discussed, with emphasis on monosymptomatic cases. Histologically Lisch nodules are formed by aggregations of oval to round cells that form dome-shaped papules on the anterior layer of the iris. Immunhistochemically these cells are characterized by positive staining with antibodies against vimentin and S-100 protein. This proves their ectodermal differentiation. Thus Lisch nodules can be seen as a direct manifestation of neuroectodermal disturbances in neurofibromatosis I.
