Zusammenfassung

Bei etwa einem Drittel der Patienten manifestiert sich die Psoriasis bereits im Kindes- und Jugendalter. Die Diagnosestellung kann aufgrund des anfangs oft untypischen klinischen Bildes und milden Befalls erschwert sein. Wie beim Erwachsenen ist auch bei Kindern die Psoriasis vom Plaquetyp am häufigsten, allerdings sind die Plaques oft kleiner und die Schuppung und Infiltration weniger ausgeprägt. Die Therapie der Psoriasis stellt bei Kindern und Jugendlichen eine besondere Herausforderung für den behandelnden Dermatologen dar, insbesondere weil nur wenige der zur Verfügung stehenden topischen und systemischen Therapeutika zur Behandlung der Psoriasis in dieser Altersgruppe zugelassen sind und Leitlinien weitgehend fehlen. Neben Psoriasistyp, Schweregrad sowie Alter des Patienten müssen Sicherheitsaspekte und Praktikabilität der Behandlung in die Therapieentscheidung einfließen. Dieser Beitrag gibt einen Überblick über die Besonderheiten der klinischen Manifestationen und der Behandlung von Kindern und Jugendlichen mit Psoriasis.

Abstract

Psoriasis starts in the first two decades of life in about one-third of patients. Initially the diagnosis may be difficult because of atypical or mild involvement. Plaque psoriasis is the most frequent type in children, just as in adults. However, lesions are often smaller, thinner and less scaly. Treatment of childhood psoriasis represents a special challenge to dermatologists since many therapeutic agents are not approved and guidelines are generally lacking for this age group. Apart from clinical type, severity of the disease and patient’s age, the choice of treatment must be based on safety concerns and accessibility of treatment. We herein focus on the peculiarities of clinical presentations and on the management of psoriasis in children and adolescents.
