Zusammenfassung
Die disseminierte superfizielle aktinische Porokeratose (DSAP) ist eine seltene, genetisch heterogene Hauterkrankung. Wir berichten über den Fall einer 73-jährigen Frau, bei der die Diagnose einer DSAP mittels optischer Kohärenztomographie (OCT) und Histologie gestellt wurde. Während der letzten 4 Jahre vor Diagnosestellung entwickelte sie mehrere (prä)maligne Hautläsionen an den Unterschenkeln einschließlich aktinischer Keratosen, Plattenepithelkarzinome und M. Bowen. Die DSAP-Läsionen und aktinischen Keratosen waren resistent gegenüber lokalen Behandlungen mit Imiquimod und Retinoiden, verbesserten sich aber unter photodynamischer Therapie (PDT).
Abstract
Disseminated superficial actinic porokeratosis (DSAP) is a rare, genetically heterogeneous skin disorder. We report a case of a 73-year-old female patient who was diagnosed with DSAP by optical coherence tomography (OCT) and histology. During the last 4 years prior to diagnosis, she had developed numerous (pre)malignant lesions of the skin of the lower legs including actinic keratoses, squamous cell carcinomas and Bowen’s disease. DSAP lesions and actinic keratoses were resistant to topical treatment with imiquimod and retinoids, but improved with photodynamic therapy (PDT).
