Zusammenfassung
Hintergrund
Das perihiläre Cholangiokarzinom (Klatskin-Tumor) ist eine seltene Tumorentität mit einer trotz multimodaler Therapie bisher ungünstigen Prognose.
Ziel der Arbeit
Ziel der Studie war es, potenziell vorteilhafte histopathologische Merkmale sowie Empfehlungen für die chirurgische Therapie bei Klatskin-Tumorpatienten zu identifizieren und so gegebenenfalls das Langzeitüberleben zu verbessern.
Material und Methoden
192 Patienten, die unter einem Klatskin-Tumor litten, unterzogen sich einer potenziellen Tumorresektion zwischen den Jahren 1998 und 2008 in der Klinik für Allgemein-, Viszeral- und Transplantationschirurgie der Universitätsklinik Essen. Insgesamt 50 Patienten aus diesem Kollektiv überlebten mehr als zwei Jahre nach der Operation. Das Follow-up endete im Dezember 2013. Typ der Resektion, UICC-Stadium und histopathologische Charakteristika wurden zwischen drei Gruppen verglichen (A–C = 2–3 Jahre, 3–5 Jahre und > 5 Jahre Überlebensgruppen).
Ergebnisse
Das 5‑Jahres-Überleben in unserer Kohorte lag bei 32 %. In 16 % der Fälle überlebten die Patienten sogar mehr als 10 Jahre. Patienten mit Lymphknotenbefall (p = 0,0126) und Metastasen (p = 0,0376) zeigten das schlechteste Überleben.
Die Perineuralscheideninvasion zeigte keinen signifikanten Einfluss auf das Gesamtüberleben. Patienten der Gruppe C (Überleben > 5 Jahre) hatten jedoch die geringste Inzidenz von Perineuralscheideninvasion mit 18,75 %. Die Lobus-caudatus-Resektion zeigte eine signifikante Assoziation (p = 0,011) mit einem Langzeitüberleben von mehr als fünf Jahren.
Diskussion
Die komplette Tumorresektion in Kombination mit der Lobus-caudatus-Resektion ist assoziiert mit verbessertem Langzeitüberleben. Die Perineuralscheideninvasion scheint ein negativer Prädiktor für das Langzeitüberleben zu sein.
Abstract
Background
Perihilar cholangiocarcinoma (Klatskin tumor) is a rare tumor entity with an unfavorable prognosis despite optimal treatment.
Objectives
The aim of the study is to investigate beneficial histopathological features and recommendations for surgery in perihilar cholangiocarcinoma to improve patients’ long term survival.
Material and methods
192 patients suffering from perihilar cholangiocarcinoma underwent attempted tumor resection between 1998 and 2008 at our clinic. 50 patients survived more than 2 years. The follow-up ended in December 2013. The resection type, the UICC stage and histopathological features were compared between three groups (2–3-year, 3–5-year and > 5-year survival groups).
Results
The overall 5‑year survival rate of the study groups was 32 %, and even 16 % survived more than 10 years after surgery. Patients with lymph node positive tumors (p = 0.0126) and distant metastasis (p = 0.0376) had the poorest survival rate. Perineural invasion had no significant impact on the overall survival, but patients surviving more than 5 years had the lowest incidence of perineural invasion with 18.75 %. Caudate lobectomy was significantly (p = 0.011) associated with a survival of more than 5 years in our study.
Conclusions
Complete tumor resection with additional caudate lobe resection is associated with long-term survival. Perineural invasion seems to be a negative prognostic factor for long-term survival.
