Zusammenfassung
Das Entrapmentsyndrom der Vena poplitea stellt eine mögliche Differenzialdiagnose bei Patienten mit venösen Beschwerden dar. Die Symptome sind zumeist unspezifisch. Beinödeme, Schwellung, Schmerzen und Krämpfe in der Muskulatur sowie Spannungsgefühl insbesondere in den Waden werden angegeben. Die meist erst als tiefe Beinvenenthrombose auftretende Erkrankung sollte vor allem bei jüngeren Patienten nach Ausschluss einer Thrombophilie als Ursache an eine mechanische Behinderung im Bereich der Kniekehle denken lassen. Eine Abklärung durch eine digitale Subtraktionsangiographie unter Provokation kann nach Doppler- und Duplexsonographie zur Diagnosesicherung erfolgen. Ein operativer Eingriff bei symptomatischen Patienten mit nachgewiesenem Entrapmentsyndrom ist bei niedriger Morbidität und Mortalität zur Vermeidung arterieller und venöser Gefäßverschlüsse aus unserer Sicht vor allem bei Jüngeren empfehlenswert. Bei bislang asymptomatischen Patienten mit ausschließlich venöser Kompression schlagen wir die Verwendung des Begriffes „Entrapmentphänomen“ vor. Wir berichten über unterschiedliche Ursachen des venösen Entrapments anhand von 3 Fallberichten und geben einen Überblick über die Literatur.
Abstract
Popliteal vein entrapment must be taken in consideration in patients with symptoms of venous insufficiency. Leg edema, swelling, calf pain, and muscle cramps are all unspecific signs. Most patients thus far have presented with deep vein thrombosis or chronic venous insufficiency. Popliteal entrapment syndrome must be taken into account in younger patients in whom predisposing factors are absent and chronic calf swelling is notable. Diagnosis is easily confirmed by noninvasive stress testing with duplex imaging and pencil Doppler probe placed over the posterior tibial artery. Additionally, digital subtraction angiography with the foot in neutral and dorsi plantarflexion is recommended for arterial entrapment. Surgery is advisable for treatment and can be done without significant morbidity. In asymptomatic patients, we suggest using the term “popliteal vein entrapment phenomenon.” We describe different etiologies of popliteal vein entrapment in three cases and present a review of the literature.
