Zusammenfassung
Aufgrund unserer Demografie spielt die onkologische Behandlung alter Menschen eine immer größere Rolle. Bisher hatte das Alter in der Therapieentscheidung an sich und in der Indikationsstellung zur Operation einen entscheidenden Stellenwert. Dies hat sich durch die Verbesserung der präoperativen Diagnostik, durch sichere Operationstechniken und eine verbesserte Intensivtherapie gewandelt. In der Durchsicht der Literatur findet sich eindeutig, dass onkologische viszeralchirurgische Operationen bei alten Menschen mit der gleichen Morbidität, Mortalität und Liegedauer wie bei Jüngeren durchgeführt werden können; das Alter allein ist kein Hinderungsgrund mehr. Die Operation sollte immer anhand der Komorbiditäten und des Tumorstadiums abgewogen werden. Die Prozeduren müssen in der Hand des Behandelnden sicher sein, palliative Eingriffe müssen ihr Ziel erreichen. Eine Operation sollte bei älteren Patienten immer im onkologischen Therapiekonzept erwogen werden.
Abstract
Oncology in elderly and frail patients is a subject of increasing importance due to demographic developments in our society. So far, patient age has been highly important for the indication of oncologic operations. This paradigm needs to be changed, since preoperative diagnostics, operative techniques, and postoperative intensive care have improved substantially over the past years. Reviewing the literature, we found that oncologic operations can be performed safely with the same morbidity, mortality, and length of hospital stay as with younger patients. Pre-existing comorbidities are the most important factor in elderly patients. Procedures must be safe and palliation needs to be effective more than curative procedures. To conclude: an surgery should always be considered in the therapeutic strategy for elderly and frail patients.
