Nutzung der ICF in der medizinischen Rehabilitation in Deutschland: Anspruch und Wirklichkeit

Use of the ICF in medical rehabilitation in Germany: claims and reality

Zusammenfassung

Hintergrund

Die internationale Klassifikation der Funktionsfähigkeit, Behinderung und Gesundheit (ICF) wird in der Praxis der medizinischen Rehabilitation auf unterschiedliche Weise genutzt. Ansprüche und Erwartungen an ihre Nutzung werden von der World Health Organization (WHO) selbst, aber auch von vielen anderen Akteuren formuliert.

Ziel der Arbeit

Die Ansprüche verschiedener Akteure an die Nutzung der ICF in der medizinischen Rehabilitation sollen dargestellt und mit Beispielen aus der Praxis verglichen werden.

Material und Methoden

Zur Beantwortung der Fragestellung wurde in einer Literaturrecherche nationale und internationale Literatur aus verschiedenen Datenbanken (PubMed, REHADAT, Google Scholar) herangezogen.

Ergebnisse

Bei den untersuchten Akteuren finden sich unterschiedliche Ansprüche hinsichtlich der Nutzung der ICF. Während der Gesetzgeber und die Leistungserbringer sehr zurückhaltend und unkonkret bleiben, formulieren andere Akteure (Wissenschaft, Verbände, Betroffene, Leistungserbringer) höhere Ansprüche und fordern eine stärkere Nutzung der ICF. In der Praxis wird die ICF in Form des biopsychosozialen Modells, der gemeinsamen Sprache und vieler unterschiedlicher Adaptionen genutzt.

Diskussion

Die unterschiedlichen Ansprüche zur Nutzung der ICF verdeutlichen die Motivationslagen und Interessen der Akteure. Sowohl von der Politik als auch von den Leistungsträgern müssen jetzt Signale ausgehen, die deutlich über unverbindliche Erklärungen hinausgehen. Gleichzeitig bedarf es einer Systematisierung und Evaluation der vielen Nutzungsansätze, die vor allem bei Leistungserbringern im Gebrauch sind. Es besteht Forschungsbedarf hinsichtlich der konkreten Nutzung ICF-basierter Instrumente und der von ihnen ausgehenden intendierten und nicht intendierten Wirkungen.

Abstract

Background

The International Classification of Functioning, Disability and Health (ICF) is being used in the medical rehabilitation practice in different ways. The World Health Organization (WHO) and many other stakeholders have formulated claims and expectations for its use.

Objectives

A comparative presentation of the claims of various stakeholders for the use of the ICF with examples in current practice.

Materials and Methods

Database searches (PubMed, REHADAT, and Google Scholar) were conducted for studies concerning claims and the current use of the ICF in practice.

Results

There are different requirements regarding the use of the ICF. While lawmakers and social insurance agencies remain very cautious and vague, other stakeholders (research institutions, organizations, stakeholders, service providers) formulate higher expectations and call for greater use of the ICF. In practice, the ICF is used in the form of a bio-psycho-social model, a common language and many different adaptations.

Conclusions

The different requirements for the use of ICF demonstrate the motivations and interests of the stakeholders. Signals must now be sent both by politics and by social insurance agencies that go far beyond non-binding declarations. Furthermore it is necessary to systematize and evaluate the many use adaptations that are primarily being used by service providers. Research is needed on the concrete use of ICF-based instruments and its intended and unintended effects.

  2. In den Disability Studies wird Behinderung in erster Linie als sozial verursachtes Phänomen verstanden. Dabei nehmen die Disability Studies die Perspektive von Menschen mit Behinderung ein und untersuchen Behinderung nicht aus Fachperspektiven (Medizin, Psychologie etc.) im Sinne einer Abweichung von der Norm.

  3. In der ICF selbst wird explizit auf die Notwendigkeit der Entwicklung weiterer Systeme bzw. Instrumente für die Nutzung der ICF in der Praxis hingewiesen [1].

  4. In einer Veröffentlichung der WHO wird dieser Umstand zwar beschrieben, jedoch nicht vor dem Hintergrund der ethischen Leitlinien der ICF diskutiert [15].
