Zusammenfassung
Die variante Creutzfeldt-Jakob-Krankheit (vCJK) geht als zuvor unbekannte Prionkrankheit des Menschen auf zoonotische Übertragungen von Erregern der bovinen spongiformen Enzephalopathie (BSE) aus dem Rind zurück. Sie trat erstmals 1995/96 im Vereinigten Königreich auf. Bis heute wurden weltweit insgesamt 219 vCJK-Fälle aus sieben europäischen und vier außereuropäischen Ländern gemeldet. Die genaue Prävalenz sub- beziehungsweise präklinischer vCJK-Infektionen ist unklar. Ungeachtet der wirksamen Bekämpfung von BSE und der erfolgreichen Unterbrechung zoonotischer Übertragungswege besteht nunmehr ein theoretisches Risiko für sekundäre Übertragungen der vCJK zwischen Menschen, beispielsweise über Blut und Blutprodukte, Organe und Gewebe oder über kontaminierte chirurgische Instrumente und Medizinprodukte. Eine wahrscheinliche vCJK-Übertragung durch Blut wurde bisher in vier Fällen berichtet, in einem weiteren Fall ist die Krankheit mutmaßlich durch ein Plasmapräparat übertragen worden. Der vorliegende Beitrag gibt einen Überblick über die epidemiologische Situation bei der vCJK und beleuchtet die Überwachung, Gefährdungsabschätzung sowie das Risikomanagement dieser neuartigen, BSE-assoziierten Prionkrankheit des Menschen.
Abstract
In the wake of the bovine spongiform encephalopathy (BSE) epidemic, variant Creutzfeldt-Jakob disease (vCJD) has emerged as a previously unknown prion disease of humans. The initial occurrence of vCJD was observed in 1995/1996, and, so far, a total of 219 vCJD cases have been reported worldwide from seven European and four non-European countries. Of these, 172 cases were observed in the United Kingdom. The exact prevalence of sub- or pre-clinical vCJD infections is unclear. Despite effective measures that have been implemented against both BSE in ruminants and its transmission to humans, there is now a theoretical risk of secondary vCJD transmissions from human to human, for example via blood and blood products, organs and tissues, or contaminated surgical instruments and medical devices. Four cases of probable vCJD transmissions via blood have been described, as well as one case of secondary infection via a plasma product. This article provides an overview of the surveillance and epidemiology of vCJD and outlines public health strategies for the risk assessment and risk management of this novel BSE-associated prion disease in humans.
