Zusammenfassung
Das Projekt „Surveillance der Antibiotika-Anwendung und bakteriellen Resistenzen auf Intensivstationen“ (SARI) liefert seit 2000 Daten zu Antibiotika-Verbrauch, Resistenzraten (RR) und Resistenzdichten (RD) auf bundesweit mittlerweile 47 interdisziplinären, medizinischen und chirurgischen Intensivstationen. Der Gesamtantibiotikaverbrauch liegt durchschnittlich bei 1170 Tagesdosen (Defined Daily Doses) pro 1000 Patiententage (Pt). Die RD von MRSA ist seit 2001 gleich geblieben. Im Gegensatz dazu ist ein kontinuierlicher Anstieg an E. coli, die gegen Cephalosporine der 3. Generation resistent sind, zu verzeichnen (4,3 bzw. 1,4 resistente Erreger/1000 Pt im Jahr 2007). Die Erhebung der lokalen Daten ist für das Antibiotika- und Qualitätsmanagement auf der eigenen Intensivstation von Bedeutung. Die Analyse der gepoolten Daten erlaubt die Beobachtung von Trends und Hochrechnungen zur Antibiotika-Anwendung und Resistenzsituation in Deutschland, die für weitere Planungen von großer Wichtigkeit sein können.
Abstract
SARI (Surveillance of Antibiotic Use and Bacterial Resistance in Intensive Care Units) provides data on antibiotic use, resistance rates (RR) and resistance densities (RD) in 47 interdisciplinary, medical and surgical intensive care units in Germany. Mean total antibiotic use was 1170 defined daily doses per 1000 patient days (pd). RD of MRSA has remained stable since 2001 in contrast to a continuous increase in the RD of 3rd generation cephalosporins E. coli (4.3 and 1.4, respectively, resistant pathogens/1000 pd in 2007). Local data of antibiotic use and bacterial resistance are important for the antibiotic and quality management of the individual ICUs. Pooled data of antibiotic use and bacterial resistance allow trends and extrapolations for Germany, which are relevant for future strategies.
