Zusammenfassung

Es wird über das Vorgehen der Atemwegssicherung während der Narkoseeinleitung bei einer Patientin mit bekannt schwierigen Intubationsverhältnissen berichtet. Nach Versagen der konventionellen Intubationshilfen konnte bei der Patientin durch Einlage der neuartigen i-gel®-Einmallarynxmaske zunächst die Ventilation gesichert werden. Anschließend wurde fiberoptisch die Einsehbarkeit von Glottis und Trachea überprüft. Bei optimaler Sicht auf die genannten Strukturen konnte ein blockbarer Standardendotrachealtubus mit einem Innendurchmesser von 6 mm komplikationslos platziert werden.

Abstract

The authors report on the airway management during induction of general anaesthesia in a patient with known difficulties with intubation. After failed optimized attempts at direct laryngoscopy ventilation was secured with the new single-use laryngeal mask i-gel™. The special construction of the i-gel™ allowed the fiber optic confirmation of a good view of the glottis and trachea, allowing successful fiber optic-guided intubation of the trachea using a size 6 mm cuffed tracheal tube.
