Zusammenfassung
Fragestellung
Prototypen des LTS, eines Larynx-Tubus mit zusätzlichem ösophagealem Lumen zur Druckentlastung des Gastrointestinaltraktes, sollen auf ihre Anwendbarkeit zur Beatmung während operativer Eingriffe überprüft werden.
Methodik
Nach Zustimmung der Ethikkommission wurde bei 30 Patienten, ASA-Klasse I und II, bei elektiven orthopädischen Eingriffen nach Narkoseeinleitung mit Fentanyl und Propofol ein LTS platziert. Die Lage des LTS wurde durch Auskultation und endtidale CO2-Messung kontrolliert. Sauerstoffsättigung, Anzahl der Platzierungsversuche, Cuffdruck, Zeit bis zum ersten Tidalvolumen, intraoperatives Tidalvolumen und Beatmungsdrücke wurden registriert. Eine Befragung zu Halsschmerzen und Heiserkeit—Angabe der Schwere auf einer visuellen Analogskala (VAS)—fand 1 h, 6 h und 24 h nach OP-Ende statt.
Ergebnisse
16 Frauen und 14 Männer wurden in die Untersuchung eingeschlossen. Bei 29 Patienten (96,7%) ließ sich der LTS platzieren (dreimal im 2. Versuch), einmal war keine suffiziente Ventilation möglich. Hier und in einem Fall mit suffizienter Beatmung, aber voraussichtlicher OP-Dauer >3 h, wurde endotracheal intubiert. Die Durchschnittszeit bis zum ersten Tidalvolumen betrug 17,3 sek, beatmet wurde über durchschnittlich 74 min. Die pulsoxymetrisch gemessene Sauerstoffsättigung betrug stets ≥97%. Die Magenauskultation war in allen Fällen negativ, der mittlere Cuffdruck lag bei 73,7 cmH2O. Unter kontrollierter Beatmung mit dem Ziel eines endtidalen CO2 von 35 mmHg (Durchschnitt 36,3 mmHg) wurde ein durchschnittliches Atemzugvolumen von 579 ml erreicht, der resultierende inspiratorische Spitzendruck betrug 20,2 mbar. Minimale Blutantragungen am LTS fanden sich in 5 Fällen, Heiserkeit bestand nach 6 h bei einer Patientin, Halsschmerzen fanden sich auf gezielte Befragung nach 1 h bei einem (VAS 3), nach 6 h bei 6 (mittlerer VAS-Wert 2,7) und nach 24 h bei 3 Patienten (VAS 1,3).
Schlussfolgerungen
Der LTS eignet sich für die Beatmung während elektiver Eingriffe und lässt sich mit hoher Erfolgsrate platzieren. Postoperative Beschwerden treten mit geringer Häufigkeit und Schwere auf.
Abstract
Objective
The feasibility of prototypes of the LTS, a laryngeal tube with an additional oesophageal drain tube for pressure relief, was tested for ventilation during surgery.
Methods
After approval of the ethics committee, a LTS was placed in 30 orthopaedic patients, ASA I and II, induction of general anaesthesia with fentanyl and propofol. Position of the LTS was verified by auscultation and endtidal CO2-measurement. Oxygen saturation, number of placement attempts, cuff pressure, time until first tidal volume, tidal volume and airway pressures were registered intraoperatively. Patients were questioned about hoarseness and soar throat 1 h, 6 h and 24 hours after surgery, graded on a visual analogue scale (VAS).
Results
16 women and 14 men were investigated. In 29 patients (96.7%), the LTS was placed successfully (second attempt in three patients). In one patient, sufficient ventilation was not possible. In this case and in another patient with sufficient ventilation but estimated duration of surgery >3 h, endotrachel intubation was performed. Average time until first tidal volume was 17.3 seconds, ventilation was performed for 74 minutes. Pulse oximetric oxygen saturation was ≥97% at all times. Auscultation over the stomach was negative in all patients with an average cuff pressure of 73.7 cmH2O. During controlled ventilation aiming at an endtidal CO2 of 35 mmHg (average 36.3 mmHg), an average tidal volume of 579 ml was reached, resulting in an inspiratory peak pressure of 20.2 cmH2O. Minimal traces of blood on the LTS were found in 5 patients, hoarseness was present in one patient after 6 hours, soar throat was stated after 1 hour by one patient (VAS 3), after 6 hours by 6 (average VAS 2.7) and after 24 hours by 3 patients (VAS 1.3).
Conclusions
The LTS can be used for ventilation during elective surgery and can be placed with a high success rate. Postoperative complaints are infrequent and mild.
