Zusammenfassung
Operationsziel
Gelenkerhaltende biplanare Korrektur von leicht- und mittelgradigen Rückfuß-Varusdeformitäten durch eine Z-förmige Osteotomie des Kalkaneus.
Indikationen
Leicht- und mittelgradige subtalare Rückfuß-Varusfehlstellung im Zusammenhang mit Cavovarus-Deformitäten, Equinovarus-Deformitäten z.B. nach idiopathischem Klumpfuß, Zustand nach tarsaler Arthrodese oder OSG-Endoprothetik, Z.n. Peronealsehnenruptur.
Kontraindikationen
Supramalleoläre Varusfehlstellungen, Verkippung des Talus in der Malleolengabel bei OSG-Instabilität, Varusarthrose des OSG, schwere kontrakte subtalare Varusfehlstellungen sowie nichtrekonstruierbarer Verlust der Peronealsehnenfunktion durch Sehnenläsion oder Lähmung, die eine Korrekturarthrodese erfordern.
Operationstechnik
Über einen lateralen bogenförmigen Zugang über dem Tuber calcanei mit Freilegung der lateralen Kalkaneus-Wand wird eine Z-förmige Osteotomie des Kalkaneus angelegt und das Tuberfragment nach lateral verschoben und in der Transversalebene des Fußes gedreht, sowie in der Frontalebene durch lateralbasige Keilentnahme leicht gekippt. Die Osteotomie wird mit Kirschnerdrähten oder Schrauben fixiert. Bei höherem Korrekturausmaß ist eine prophylaktische Spaltung des Tarsaltunnels empfehlenswert.
Weiterbehandlung
Entlastung im Unterschenkel-Gipsverband für 4 Wochen. Nach Entfernung der Kirschner-Drähte weitere 4 Wochen Teilbelastung im Walker. Bei Verwendung von Schrauben sind eine frühere Mobilisierung und Teilbelastung möglich.
Ergebnisse
Bei 20 von 2009 bis 2013 durchgeführten Kalkaneus-Scarf-Osteotomien (mit zusätzlichen knöchern-weichteiligen Maßnahmen) konnte klinisch eine ausreichende Korrektur der Rückfuß-Varusfehlstellung erzielt werden. Es wurden keine Fälle von verzögerter knöcherner Konsolidierung, Korrekturverlust oder Schädigung des N. tibialis festgestellt. Eine Reoperation war bisher in keinem Fall erforderlich.
Abstract
Objective
Joint-preserving correction of mild and moderate hindfoot varus deformities by Z-shaped calcaneal osteotomy.
Indications
Mild and moderate subtalar varus deformities in conditions like cavovarus or equinovarus foot deformity, peroneal tendon rupture, malpositioned tarsal fusion, after ankle arthroplasty.
Contraindications
Supramalleolar varus deformities, ankle instability with talar tilting, varus arthritis of the ankle joint, severe subtalar arthritis, and complete loss of peroneal muscle function that should be treated by fusions.
Surgical technique
Lateral curved incision at the tuber calcanei with exposure of the lateral calcaneal wall. Z-shaped osteotomy of the calcaneal bone with lateral displacement and rotation in the transversal plane of the tuber fragment combined with lateral wedge resection for frontal plane correction. Fixation of the osteotomy with K-wires or screws. In case of extended correction, prophylactic tarsal tunnel release is advisable.
Postoperative management
Cast without weight-bearing for 4 weeks. After removal of K-wires, 4 weeks with partial weight-bearing in a walker. In case of screw fixation, time of immobilization and nonweight-bearing can be shortened.
Results
Since 2009, 20 cases have been performed in the described technique by the author. In addition, all patients had simultaneous bony and soft tissue surgery. Residual deformity or overcorrection, delayed bony healing or tibial nerve damage were not observed. No patient has undergone revision surgery.
