Zusammenfassung
Operationsziel
Geschlossene Reposition und Fixation dislozierter proximaler Radius- oder Ulnafrakturen mittels elastischstabiler intramedullärer Nagelung (ESIN); Vermeidung von Durchblutungsstörungen im Rahmen offener Repositionen und daraus folgender funktioneller Einschränkungen.
Indikationen
Proximale Radiusfrakturen mit > 45° Abkippung (< 10. Lebensjahr) oder > 20° (> 10. Lebensjahr); proximale extraartikuläre Ulnafrakturen mit > 10° Achsenfehler; Kombinationsverletzungen (Monteggia, MonteggiaÄquivalent).
Kontraindikationen
Minimal oder nicht dislozierte Frakturen.
Operationstechnik
Radius: Einbringen eines intramedullären Nagels von distal; Fassen des proximalen Radiusfragments mit der Nagelspitze, ggf. nach digitaler oder perkutaner Repositionshilfe (Kirschner-Draht); Zurückführen des Fragments in die korrekte Position durch Drehen des Nagels.
Ulna: Einbringen eines vorgebogenen intramedullären Nagels von distal und Platzierung der Vorbiegung gegenläufig zur Fehlstellung.
Weiterbehandlung
Funktionelle Nachbehandlung, keine zusätzliche äußere Ruhigstellung, keine Physiotherapie.
Ergebnisse
78–87% sehr gute und gute funktionelle Ergebnisse.
Abstract
Objective
Closed reduction and fixation of displaced proximal radial neck or ulnar fractures with elastic stable intramedullary nailing (ESIN); prevention of circulatory disturbances and functional deficits following open reduction.
Indications
Proximal radial neck fractures > 45° displacement (< 10 years of age) or > 20° (> 10 years of age); proximal extraarticular ulnar fractures > 10° malalignment; combination injuries (Monteggia fracture, Monteggia-equivalent injury).
Contraindications
Minimally displaced or undisplaced fractures.
Surgical Technique
Radius: distal implantation of the nail; touching and fixation of the proximal fragment with the tip of the nail, if need be following digital or percutaneous partial reduction (Kirschner wire); definitive reduction by torsion of the nail.
Ulna: distal implantation of a prebent nail; the prebent curve is placed opposite to the malalignment.
Postoperative Management
Functional treatment, no additive immobilization, no physical therapy.
Results
78–87% very good and good functional results.
