Zusammenfassung
Läsionen, die aus einer Störung der normalen kortikalen Entwicklung resultieren und umgangssprachlich häufig mit dem Begriff kortikale Dysplasien zusammengefasst werden, sind derzeit neuroradiologisch und neuropathologisch uneinheitlich klassifiziert. Eine von Barkovich und Mitarbeitern erarbeitete neuroradiologische Klassifikation bezieht pathogenetische Überlegungen und genetische Befunde ein und unterscheidet Läsionen, die aus Störungen der Proliferation, der Migration oder der Organisation des Kortex resultieren. Verschiedene neuropathologische Klassifikationen berücksichtigen die Schichtung des Kortex und das Vorhandensein unreifer Neurone, von Riesenneuronen, dysplastischen Neuronen und Ballonzellen. Einige Läsionen (kortikale Dysplasie mit Ballonzellen, tuberöse Sklerose, Hemimegalenzephalie, fokale Polymikrogyrien) sind neuroradiologisch und neuropathologisch gut korreliert. Weil diese Läsionen selten operiert werden, werden bei Lissenzephalien, Heterotopien und bilateralen Polymikrogyrien entsprechende Korrelationen auch zukünftig kaum zu erhalten sein. Eine Aufgabe wird zukünftig in der besseren Korrelation von fokalen kortikalen Dysplasien ohne Ballonzellen und von Schichtungsstörungen des Kortex liegen.
Abstract
Different neuroimaging and neuropathological classifications for malformations of cortical development (or cortical dysplasias) exist. A neuroimaging classification of Barkovich and coworkers considers pathogenic and genetic features and distinguishes lesions due to disturbances of proliferation, migration and cortical organization. Neuropathological classifications are based on the presence of cortical dyslamination, immature neurons, giant neurons, dysplastic neurons and balloon cells. There is a good correlation for some lesions (focal cortical dysplasia with balloon cells, tuberous sclerosis, hemimegalencephaly, focal polymicrogyrias). Other lesions (lissencephalies, heterotopias, bilateral polymicrogyrias) are rarely operated and scarcely correlated. A major task in the near future is to better correlate focal cortical dysplasia without balloon cells and cortical lamination disorders.
