Abstract

Woven coronary artery is an extremely rare and not clearly defined congenital malformation in which the epicardial coronary artery is divided into multiple thin channels. These multiple channels merge again into a normal conduit. This anomaly is a benign condition because there is normal blood flow after the anomalous segment of the abnormal coronary artery. We describe a woven coronary artery anomaly in the left anterior descending artery (LAD) and its first diagonal (DI) branch causing myocardial infarction that was confused with coronary dissection. The definitive diagnosis was made by optical coherence tomography (OCT). Although woven coronary is a benign anomaly, it may cause myocardial infarction. We used the novel method of OCT for the definitive diagnosis and appropriate treatment of this anomaly.

Zusammenfassung

Die Fehlbildung der“woven coronary artery” ist äußerst selten und stellt eine kongenitale Malformation dar, bei der die Koronararterie sich in viele dünne Gefäßkanälchen aufspaltet. Diese Gefäßkanälchen verschmelzen dann wieder zu einem normalen Gefäß. Diese Anomalie wird als benigne Erkrankung angesehen, da hinter dem anomalen Segment der betroffenen Koronararterie ein normaler Blutfluss besteht. Hier wird eine solche Anomalie im Ramus interventricularis anterior (LAD) und seinem ersten schräg abgehenden Ast (DI) beschrieben, die einen Herzinfarkt verursachte, welcher für eine Koronararteriendissektion gehalten wurde. Die endgültige Diagnose erfolgte durch optische Kohärenztomographie (OCT). Auch wenn die „woven coronary artery“ eine benigne Anomalie darstellt, kann sie doch einen Herzinfarkt verursachen. Hier wurde ein neues Verfahren, die OCT, eingesetzt, um die endgültige Diagnose zu stellen und so eine geeignete Behandlung dieser Anomalie einleiten zu können.
