Zusammenfassung
Trotz deutlicher Verbesserungen der Therapie der akuten Aortendissektion (AAD) ist die Letalität in der Initialphase der Erkrankung inakzeptabel hoch. Rechtzeitige Diagnose und Therapie sind die dominierenden Faktoren zur Verbesserung der Prognose der AAD. Wesentliche Vorbedingung für die korrekte, rechtzeitige Diagnose ist der Bewusstseinsimpuls, nämlich das „Darandenken“. Verzögerte oder gar Fehldiagnosen haben nicht selten katastrophale Folgen.
Häufig erlauben die geklagten Brust- oder Rückenschmerzen, mit dem für eine AAD typischen zerreißenden, stechenden Charakter und hoher Intensität, verbunden mit der Physiognomie eines Marfan-Syndroms bereits die Verdachtsdiagnose AAD, um die sofortige anschließende Bildgebung und Therapie zu veranlassen. Für weniger klare Fälle werden zusätzliche Hinweise aus der Anamnese und spezielle Befunde der ärztlichen Untersuchung in einem Diagnosepfad schematisch zusammengestellt. Als innovative Form der Diagnostik wird das präventive echokardiographische Screening in Risikogruppen angesprochen.
Um die Sensibilität für das Erkennen dieser lebensbedrohlichen Krankheit zu erhöhen, ist die Plakatkampagne „Daran denken kann Leben schenken“ initiiert worden. Das Plakat zeigt schematisch die Aortendissektion, deutet auf das Marfan-Syndrom als einen wichtigen Risikofaktor in jugendlichem Alter hin und zeigt eine CT-Aufnahme als Hinweis auf die am häufigsten durchgeführte Untersuchungsmethode mit hoher Sensitivität und Spezifität.
Abstract
Despite significant improvements in the surgical therapy of acute aortic dissection (AAD), mortality rates in the initial phase remain unacceptably high. Early diagnosis and therapy are essential to improving prognosis in these patients. A prerequisite of prompt and correct diagnosis is“thinking of it”. Delayed or incorrect diagnosis can often have catastrophic results.
The reported acute chest and back pain of a tearing, stabbing nature combined with the physiognomy of Marfan syndrome often arouse the clinical suspicion of AAD, prompting immediate imaging of the thoracic aorta and therapy. For less clear cases, additional hints drawn from the patient history and special findings from the medical examination are presented schematically in a diagnostic pathway. As an innovative form of diagnosis, preventive echocardiographic screening in high risk groups is discussed.
To heighten awareness of AAD and the importance of its correct diagnosis, the poster campaign “Thinking of it can save lives” has been initiated. The poster depicts AAD schematically, indicates Marfan syndrome as a risk factor for AAD in young people and illustrates a CT scan as the most frequently performed imaging technique with high sensitivity and specificity.
