Zusammenfassung
Die sog. Pelvic Sepsis ist eine multiätiologische lebensbedrohliche Infektion im Anorektal- und Urogenitalbereich. Die sich ausbreitenden Nekrosen stellen eine Sonderform der nekrotisierenden Weichteilinfektionen („necrotizing soft tissue infections“, NSTI) dar. Die Erkrankung beginnt mit einer perianalen oder urogenitalen Infektion; Traumata oder postoperative Komplikationen können ebenso die Ursache sein. Im Gegensatz zum lokalisierten Abszess liegt eine diffuse Ausbreitung vor, die längs der Faszienstrukturen und der durch sie bestimmten Spalträume der Beckenregion verläuft. Aufgrund der rasanten Ausbreitung der Infektion besteht bereits bei Diagnosestellung ein septisches Zustandsbild. Das klinische Bild bestimmt die Indikation zur sofortigen chirurgischen Intervention mit ausgedehnter Exzision aller betroffenen Areale. Nur durch die unmittelbare chirurgische Intervention kann die weiterhin hohe Letalität dieser Erkrankung reduziert werden. Bei polymikrobiellen Infektionen mit gramnegativen, grampositiven Erregern und Anaerobiern sollte die Therapie mit einem Breitbandantibiotikum, wie z. B. mit einem Kombinationspräparat Piperazillin/Tazobactam, erfolgen. In die interdisziplinäre Therapie sind im unmittelbaren Verlauf Intensivmediziner und zur Rekonstruktion der bestehenden Defekte plastische Chirurgen einzubeziehen.
Abstract
The term “pelvis sepsis” describes a destructive and life-threatening process in the perianal and genital areas. The rapidly spreading necrosis is a special form of necrotizing soft tissue infection (NSTI). Most patients initially have perianal or urinary tract infections but trauma or postoperative complications may also be a cause of the disease. In contrast to a localized abscess the infection spreads diffusely along the fascial planes into the whole pelvic region. Due to the rapid progress the infection evolves into a systemic sepsis in the early phase of the illness. The clinical presentation makes a radical debridement of the involved areas necessary. Only prompt surgical intervention can reduce the high mortality of the disease. Most cases are caused by mixed Gram negative, Gram positive and anaerobic flora. Empirical antimicrobial treatment should be started with a broad spectrum antibiotic combination, e.g. piperacillin and tazobactam. Interdisciplinary management is essential and should include critical care physicians and reconstructive plastic surgeons.
