Development of a New Scoring System for the Evaluation of Incontinence and Constipation in Children

Abstract

Background and Purpose: In the diagnosis of bowel function disorders, scoring systems are of great value.However, the scores in common use are often inadequately suitable for children.Against this background, a simple-to-apply questionnaire-based scoring system for the diagnosis of incontinence, constipation and combinations of the two was specially developed for use in children.

Material and Methods: With this Pediatric Incontinence/Constipation Score (PICS), 122 children were investigated with regard to their bowel function.

Results: On the basis of the data obtained in this way, normal curves for the degree of incontinence and constipation in childhood were established.The incontinence scores were found to correlate significantly with age,whereas constipation scores were independent of age.

Conclusions: The standard curves for incontinence and constipation obtained with this new scoring system make it possible to evaluate bowel function in children, and to monitor its course.

Zusammenfassung

Hintergrund: Bei der Diagnostik von Stuhlverhaltensstörungen spielen Scoring-Systeme eine große Rolle. Jedoch sind die gebräuchlichen Scores für Kinder oftmals nur bedingt geeignet und beurteilen nur eines der Merkmale von Stuhlverhaltensstörungen.

Material und Methodik: Insbesondere für die Diagnostik von Inkontinenz, Obstipation und deren Mischformen im Kindesalter wurde mit Hilfe eines Fragebogens ein Scoring-System entwickelt, das klinisch einfach anzuwenden ist. Mit diesem “Inkontinenz-Obstipations-Score für Kinder” (“IOS für Kinder”) wurden 122 Kinder bezüglich ihres Stuhlverhaltens befragt.

Ergebnisse: Hierbei konnten Normkurven für die Ausprägung von Inkontinenz und Obstipation im Kindesalter erstellt werden.Die Inkontinenz-Scorewerte waren signifikant altersabhängig, während die Obstipations-Scorewerte unabhängig vom Alter waren.

Schlussfolgerungen: Mit den erstellten Normkurven für Inkontinenz und Obstipation ist es möglich, das Stuhlverhalten von Kindern zu bewerten und den Verlauf zu kontrollieren.
