Mirizzi syndrome

Abstract

In recent years there have been several case reports of patients suffering from the so-called Mirizzi syndrome. Personal experience with 5 patients, together with published data from 19 others, is used to consider whether this syndrome is a true entity. It is concluded that the involvement of the hepatic duct in gallstone disease may result in a wide range of lesions of differing severity with no common pattern. Consequently, the term “Mirizzi syndrome” has no well-defined meaning and should be abandoned.

Résumé

Au cours de ces dernières années plusieurs cas de syndrome de Mirizzi ont été publiés. Notre expérience personnelle concernant 5 malades, associée à celle fournie par 19 cas colligés dans la littérature, nous a permis d'étudier cette entité.

Nous pensons qu'elle ne répond pas à la réalité et que les lésions qui caractérisaient ce syndrome sont dues simplement à l'action des calculs biliaires sur le canal hépatique.

Resumen

El concepto del sindrome de Mirizzi, según la propuesta original del autor, se fundamenta en la presencia de un mecanismo esfintérico funcional en el canal hepático como consecuencia de colelitiasis. En años recientes se han informado varios casos de pacientes con el denominado “sindrome de Mirizzi”. La experiencia personal con5 pacientes, junto con el análisis de otros 19 casos publicados, es utilizada para determinar si este síndrome representa una entidad clínica real. Se concluye que la afecci'on del canal hepático por enfermedad litiásica de la vesícula biliar puede resultar en una amplia variedad de lesiones de severidad alterable, pero sin un patrón patológico común. En consecuencia, consideramos que el “síndrome de Mirizzi” no conlleva un significado definido y que tal denominación debe ser abandonada.
