Abdominale und thorakale Organverletzungen

Zusammenfassung

Arbeitsunfälle mit Verletzung von Thorax und Abdomen werden regelhaft durch stumpfe Traumen hervorgerufen. Dies hat Konsequenzen für die Diagnostik und die Therapie im Vergleich zu penetrierenden Verletzungen. Das primäre Ziel der Behandlung von schweren Verletzungen von Thorax und Abdomen ist der Erhalt der Vitalfunktionen. Das Management kann in 4 sequenzielle Phasen eingeteilt werden: lebensrettende Sofortmaßnahmen, Bewertung der Läsion, primäres Management und definitive Versorgung. Zumindest die ersten 3 Phasen gehören im Sinn einer „damage control“ in das Repertoire des Traumatologen, um das spezielle Organmanagement zu ermöglichen. Letzteres ist vor dem Hintergrund der fortschreitenden Spezialisierungen der operativen Fächer heute zunehmend als interdisziplinäre Versorgung zu verstehen, die es ermöglicht, Komplikationsraten zu minimieren und ein optimales Behandlungsresultat zu erreichen. Dies gilt insbesondere für komplexe Organverletzungen und deren Folgen. Die vorliegende Arbeit soll in einer Übersicht das derzeitige operative Spektrum in der Behandlung von Organverletzungen an Thorax und Abdomen vorstellen.

Abstract

Occupational accidents leading to thoracic and abdominal lesions are generally caused by blunt trauma. This has implications for both the diagnostic and the therapeutic algorithms applied, which differ from those used for perforating wounds. The primary goal of treatment of severe thoracic and abdominal lesions is to preserve vital functions. The management is divided into four sequential phases: resuscitation, evaluation, initial management, and definitive treatment. At least the first three phases can be regarded as aspects of damage limitation, and as these such fall in the trauma surgeon’s area of responsibility; not until they are completed will the specific management of the organ lesions sustained be possible. As a result of the increasingly complex specialization in surgery, this last is now regarded as an interdisciplinary treatment that can keep complication rates low and yield optimal treatment results, particularly in the case of complex organ lesions and their sequelae. This paper reviews the current spectrum of surgical procedures applied in the treatment of organ lesions sustained in occupational accidents involving thoracic and abdominal trauma.
