Zusammenfassung
Bei den gewerblichen Unfallversicherungsträgern werden jährlich > 50.000 Unfälle im Straßenverkehr mit Distorsion der Halswirbelsäule (HWS) gemeldet. Nur etwa 5% der Fälle bedürfen stationärer Behandlung. Bei der ambulanten Behandlung stehen Zerrungen der HWS im Vordergrund. Solche Zerrungen stellen versicherungsrechtlich einen Körperschaden dar. Bei adäquater Behandlung tritt kurzfristig wieder Arbeitsfähigkeit ein. Überbehandlung, insbesondere eine längere, unkontrollierte Stilllegung des Halses, kann zu Folgeschäden besonders an der Muskulatur führen, die erst den Versicherungsfall begründen. Gelingt es, die betroffenen Versicherten in der Frühphase adäquat zu betreuen, wird die Zahl der zunächst therapieresistenten Patienten nach einer Distorsion der HWS klein bleiben. Die betreffenden Patienten sollten baldmöglichst nach dem Unfall einer besonders intensiven Diagnostik und fachübergreifenden Betreuung zugeführt werden. Das kann nur bei einer guten Kommunikation zwischen Versicherten, Ärzten und Versicherungsträgern erreicht werden.
Abstract
The industrial injuries insurance firms receive notification of over 50,000 street traffic accidents involving distorsion of the cervical spine every year. Only about 5% of these cases need in-patient hospital treatment. Strains of the cervical spine are among the most frequent conditions requiring out-patient treatment. Such a strain is a physical injury as defined in insurance law. Correct treatment leads to restored fitness for work within a short time. Overtreatment, especially a long period of unmonitored immobilization of the neck, can lead to secondary damage, particularly to the muscles, which then itself becomes the reason for seeing the case as an event covered by insurance. If the insured persons receive adequate care early on, the number of initially therapy-resistant patients after distorsion of the cervical spine will remain low. Patients should undergo particularly intensive diagnostic procedures and receive multidisciplinary care as soon as possible after the accident. This can only be achieved if there is a good system of communication between insured persons, doctors, and insurance institutions.
