Zusammenfassung
Die Morbidität der ventralen Zugänge hat die Entwicklung therapeutischer Konzepte zur Behandlung von Verletzungen der Rumpfwirbelsäule seit Beginn der Ära der Wirbelsäulenchirurgie wesentlich beeinflusst. Obwohl das Konzept der rein dorsalen Aufrichtung und Stabilisierung die in es gesetzten Erwartungen letztendlich nicht erfüllen konnte, blieben ventrale Eingriffe eher die Ausnahme und der operativen Behandlung größerer posttraumatischer Destruktionen und Defektbildungen am Wirbelkörper vorbehalten. Minimalinvasive Verfahren, in endoskopischer oder reduziert invasiver Technik vorgenommen, und nicht zuletzt die Entwicklung speziell für diese Verfahren konzipierter Implantate haben neue Wege in der Traumabehandlung der Wirbelsäule aufgezeigt. Über einen 5-Jahres-Zeitraum wurden mehr als 300 konsekutive Verletzungen der Brust- und Lendenwirbelsäule unter thorakoskopischen Bedingungen versorgt. Die Analyse der Serie zeigt eine niedrige intraoperative Komplikationsrate und eine im Vergleich zum offenen Verfahren deutlich geringere postoperative Morbidität der Patienten.
Abstract
Since the early days of spinal surgery, the development of therapeutic concepts for the treatment of injuries to the thoracic and lumbar spine has been heavily influenced by the morbidity attaching to the anterior approaches. Although the concept of a single dorsal reduction and stabilization did not live up to expectations, for a long time the use of ventral procedures remained the exception and they were reserved mainly for larger scale posttraumatic destruction and defects of the vertebral body. Minimally invasive surgery of the spine, performed endoscopically or with less invasive techniques, and not least the development of implants designed specifically for use in this kind of surgery, have highlighted new approaches to treating injuries to the spine. Over 5 years more than 300 consecutive unstable injuries of the thoracic and lumbar spine have been treated operatively by endoscopic techniques. Analysis of the series reveals a low rate of intraoperative complications and a significantly lower postoperative morbidity rate than in patients treated by the open procedure.
