Zusammenfassung
Die meist auf ein Hochrasanztrauma zurückgehenden Verrenkungsbrüche des Chopart-Gelenks sind schwere Fußverletzungen, deren Auswirkung auf die dynamische Fußfunktion, aufgrund ihrer Seltenheit und der häufig schweren Begleitverletzungen, vielfach unterschätzt wird. Da die Wiederherstellung einer ungestörten Fußfunktion in der Regel nur durch eine anatomische Rekonstruktion der Gelenkebene und Wiederherstellung der Länge von medialer und lateraler Fußsäule erreicht werden kann, kommt der exakten präoperativen Evaluation des Gesamtverletzungsausmaßes prognostisch eine entscheidende Bedeutung zu. Beim Verdacht auf geringfügige Dislokationen oder unverschobene Frakturen des Chopart-Gelenks wird die Durchführung einer Computertomographie empfohlen, da es häufig erst dadurch gelingt, zusätzliche Verletzungen, wie zentrale Trümmerzonen, Subluxationsstellungen oder knöcherne Band- bzw. Sehnenausrisse, darzustellen. Die Prinzipien der operativen Therapie von Chopart-Luxationsfrakturen beinhalten die biomechanisch entscheidende Wiederherstellung der 2-Säulen-Statik des Fußes, die Rekonstruktion der Gelenkebenen sowie das Erkennen und die Therapie von ligamentären Instabilitäten.
Abstract
Dislocation fracture of the transverse tarsal (Chopart’s) joint is a rare condition that is frequently misdiagnosed. Early recognition and treatment is needed to avoid instability and significant long-term foot problems. The clinician with a general knowledge of the various injury patterns to the midfoot is able to approach these injuries rationally and with an appreciation of their potential severity. The necessity for routine antero-posterior, lateral and oblique radiographs is emphasized. Routine CT scanning is recommended for detection of any occult injuries such as central compression zones and ligament or tendon avulsion. The classification and treatment of midtarsal joint injuries and midfoot fractures are discussed. Open reduction with internal fixation is recommended for displaced fractures. The primary goals of treatment are restoration of the length of the ¶medial and lateral columns of the foot and restoration of joint congruity and midfoot stability.
