Kraniofaziale Techniken in der Traumatologie

Zusammenfassung

Der Bügelschnitt bietet als operativer Zugang eine gute Übersicht und ermöglicht die anatomisch gerechte Reposition der Fragmente bei komplexen Mittelgesichts- und Schädelbasisfrakturen. Durch diesen Zugang ist der gleichzeitige Einsatz kraniofazialer Techniken wie Marginotomie, Lidbandfixation und Knochenentnahme möglich. Die Primärversorgung kann dadurch optimiert und Sekundäreingriffe können reduziert werden. Wir haben unsere nach diesen Techniken versorgten Patienten bezüglich funktionellen und ästhetischen Gesichtspunkten nachuntersucht. Die Komplikationsrate war gering, der Vorteil jedoch sehr groß, so daß wir der Meinung sind, daß die Indikation für dieses operative Vorgehen großzügiger gestellt werden sollte.

Summary

The coronal incision is a versatile approach to midface or skull base fractures. It allows the open reduction of complex fracture sites and facilitates canthopexy, marginotomy or calvarian bone harvesting. By this method, primary treatment can be optimised and the rate of secondary corrective surgery can be reduced. All trauma patients who underwent this kind of intervention were re-examined to check functional and aesthetic results. The complication rate was low and the advantages of these techniques are evident, so that it is suggested that the indication for this operative approach should be extended.
