Summary.
Right hemihepatectomy is an anatomically based major resection technique for primary and metastatic liver cancer. The most dangerous step in the procedure is to mobilize the right liver lobe from the retroperitoneum and the inferior vena cava. Large tumors or those with extension to the diaphragm, adrenal gland, or kidney may be more easily resected if a frontal approach to dividing the liver is followed. After dissection of the hilus, the liver is divided and during this step all vessels draining to the vena cava are separated. Then, the right liver lobe is mobilized together with surrounding structures if necessary, but with no significant risk of bleeding. Our experience in four patients operated on for metastases or different primary tumors demonstrates that this frontal approach may be safely used and allows minimal manipulation of the tumor. Intraoperative ultrasound may be helpful in the detection of the middle and right hepatic vein.
Zusammenfassung.
Bei der Hemihepatektomie rechts wegen primärer Lebercarcinome oder Metastasen ist die Mobilisation des rechten Leberlappens aus dem Retroperitoneum und zur V. cava der schwierigste Schritt. Für voluminöse Tumoren des rechten Leberlappens oder solche mit Infiltration in Nebenniere, Niere oder Zwerchfell bietet sich eine alternative Resektionstechnik an. Nach Dissektion des Leberhilus wird zunächst von frontal das Leberparenchym bis auf die V. cava durchtrennt. In die V. cava drainierende Gefäße werden unter optimaler Sicht versorgt. Erst danach erfolgt im Status des von allen wesentlichen Gefäßen befreiten rechten Leberlappens dessen Mobilisation unter Einschluß infiltrierter Umgebungsstrukturen. Die Erfahrungen bei 4 Patienten (Alter 58–65 Jahre) mit Metastasen unterschiedlicher Primärtumoren zeigt, daß dieses Verfahren sicher, übersichtlich und mit minimaler Manipulation am Tumor durchführbar ist. Der intraoperative Ultraschall kann als Lokalisationshilfe für die rechte bzw. mittlere Lebervene dienen.
