Zusammenfassung
In einer sozio-ökonomisch und beruflich homogenen Gruppe von 163 stellvertretenden Meistern (“Vizemeistern”) eines industriellen Grossunternehmens (40–55 Jahre; 48,4±4,5) wird der Zusammenhang zwischen psychosozialen Arbeitsbelastungen und Zigarettenrauchen analysiert. Psychosoziale Arbeitsbelastungen werden anhand des Modells beruflicher Gratifikationskrisen gemessen. Die Wahrscheinlichkeit, zur Gruppe der regelmässigen Raucher zu gehören, ist bei Vorliegen hoher Gratifikationskrisen mehr als viermal so hoch wie bei Fehlen dieser Belastungen (odds ratio nach Kontrolle des Alterseffektes: 4,34;95%CI1,50–12,54). Der Effekt bleibt auch in der höheren Altersgruppe bestehen, obwohl der Anteil von Rauchern mit zunehmendem Alter sinkt. Innerhalb der Gruppe der Raucher gehören zwar 50% aller Vizemeister, die unter hohen beruflichen Gratifikationskrisen leiden, zu starken Rauchern (≧20 Zigaretten pro Tag), gegenüber 28% bei den gering Belasteten, jedoch lässt sich dieser Befund aufgrund der geringen Fallzahl statistisch nicht absichern. Zusammenfassend belegen die Daten einen Zusammenhang zwischen erfahrenen, beruflichen Gratifikationskrisen und dem Risiko regelmässigen Zigarettenrauchens in einer beruflich und sozio-ökonomisch homogenen Gruppe abhängig Beschäftigter des mittleren Erwachsenenalters.
Résumé
L'association entre le stress psychosocial au travail et le tabagisme est analysée dans un groupe de cadres moyens dans l'industrie (n=163; 40–55 ans; 48,4±4,5) homogène en termes de profession et de couche sociale. Le stress psychosocial est analysé par rapport au modèle de crise de gratification (déséquilibre entre ce que l'on fait et ce que l'on obtient). La fréquence du tabagisme est plus de quatre fois plus haute dans le groupe souffrant de crises de gratification que dans le groupe nonexposé (odds ratio après contrôle de l'effet de l'âge: 4,34; 95% CI 1,50–12,54). Cet effet est observé même dans le groupe le plus âgé, quoique la fréquence du tabagisme diminue lorsque l'âge augmente. Si l'on considère le groupe des fumeurs, on observe que 50% de ceux qui souffrent des crises de gratification sont des fumeurs forts (≧20 cigarettes par jour), mais seulement 28% de ceux moins exposés au stress psychosocial (association non significative à cause des petits nombres). En conclusion, une association entre crise de gratification au travail et tabagisme a été démontrée dans un groupe de cadres moyens de l'industrie. Cette association ne peut pas être attribuée à l'influence de l'âge, de type de profession ou de couche sociale.
Summary
The association between psychosocial work stress and cigarette smoking is analyzed in a socioeconomically and professionally homogeneous group of 163 middle managers (40–55 years; 48.4 ±4.5) in a large industrial company. Psychosocial stress is defined in terms of an imbalance between effort spent and reward obtained at work. The relative risk of regular smoking is 4.34 (odds ratio after controlling for age; 95% CI 1.50–12.54) in those middle managers who suffer from a marked imbalance between effort and reward, compared to those who are free from this type of psychosocial stress at work. This effect persists in the older age group although the proportion of smokers decrease with age. Within the group of regular smokers, every second middle manager suffering from a marked imbalance between effort and reward is a heavy smoker (≧20 cigarettes/day) whereas in the group experiencing less stress at work, only 28% heavy smokers are found. However, due to small numbers this difference is not statistically significant. In conclusion, the data reported in this study demonstrate an association between psychosocial stress at work in terms of high effort and low reward and the risk of regular smoking. This association is not confounded by age, socio-economic status or type of occupation.
