Riassunto
Scopo: Proporre una revisione aggiornata della letteratura riguardo a epidemiologia, diagnosi, sottotipi, comorbilità, trattamento e prognosi del disturbo di identità di genere (DIG). Materiali e metodi: Revisione della letteratura su questo argomento. Risultati: Il transessualismo è una condizione caratterizzata da un’intensa e persistente identificazione col sesso opposto, in persone che non presentano alcuna anomalia fisica. Può riguardare sia il sesso femminile (disturbo da femmina a maschio, FtM) che quello maschile (disturbo da maschio a femmina, MtF) con una sex ratio M:F di circa 3:1. L’eziologia del transessualismo è ancora in gran parte sconosciuta. L’esame obiettivo, i dosaggi ematici ormonali e il cariotipo possono soltanto confermare il sesso biologico. Recenti osservazioni sembrano indicare una differenza anatomica/funzionale cerebrale tra transessu ali e non transessuali. Sembra, inoltre, che abbiano una certa importanza anche fattori ambientali precoci. Le linee guida degli Standards of Care dell’International Harry Benjamin Gender Dysphoria Association (ora chiamato World Health Professional Association for Transgender Health, WPATH) raccomandano di affrontare il percorso che porta alla riassegnazione chirurgica di sesso (RCS) in due fasi. La prima fase consiste nella valutazione diagnostica, secondo i criteri categoriali psichiatrici specificati nel Diagnostic and Statistical Manual of Mental Disorders — Fourth Edition — Text Revision (DSM-IV-TR). Nella seconda fase, viene valutata la capacità del soggetto di vivere quotidianamente secondo il ruolo di genere desiderato. In questa seconda fase viene somministrata la terapia ormonale e la psicoterapia, pur non essendo obbligatoria, è fortemente consigliata. Nonostante le casistiche siano limitate, sembra che le persone transessuali adeguatamente trattate abbiano una migliore prognosi se paragonate a coloro che non arrivano all osservazione medica. Conclusioni: Il transessualismo non è un fenomeno omogeneo e ciò dovrebbe essere tenuto in considerazione sia nella ricerca che nella pratica clinica.
