Deze beschouwing gaat in op de rol van de huisarts bij patiënten die ingrijpende gebeurtenissen hebben meegemaakt (ongevallen, rampen, oorlogen, berovingen, inbraken, fysieke mishandeling en seksueel misbruik).
Tweeënveertig procent van de patiënten uit eigen onderzoek in een Limburgse huisartsgeneeskundige populatie heeft ooit een of meer ingrijpende gebeurtenissen meegemaakt. Hun huisartsen zijn op de hoogte van 7,9% van die gebeurtenissen.
Patiënten willen vooral steun en begrip, maar ook initiatief van de huisarts bij opsporing en follow-up. Er dient voorlichting gegeven te worden over typische symptomen na een ingrijpende gebeurtenis, zoals herbeleving, vermijding en verhoogde prikkelbaarheid. De huisarts moet afwegen of deze symptomen tot het normale verwerkingsproces behoren, dan wel wijzen op een posttraumatische stressstoornis. Het is ook belangrijk dat huisartsen deze ingrijpende gebeurtenissen noteren. Het blijkt moeilijk patiënten met PTSS-klachten te verwijzen.
Een nieuwe bevinding is dat PTSS-symptomen evenzeer optreden bij bovenstaande ingrijpende gebeurtenissen als bij levensgebeurtenissen zoals echtscheiding en werkloosheid.
