Passagère cognitieve achteruitgang bij een patiënt met relapsing polychondritis

Samenvatting

Patiënt A, 58 jaar, wordt naar de geriatrie verwezen door de reumatoloog in verband met apathie. Anamnese vermeldt verspringende gewrichtspijnen, rode ontstoken ogen, apathie en interesseverlies. Lichamelijk onderzoek toont rode tranende ogen, bradyfrenie en bradykinesie. Bloedonderzoek laat verhoogde infectie parameters zien. De patiënt ontwikkeld koorts en bilateraal rode ontstoken oren. Diagnose van relapsing polychondritis wordt gesteld, de patiënt wordt behandeld met prednison en de symptomen verdwijnen.

Relapsing polychondritis is een chronische ziekte geassocieerd afbraak van structurele en functionele integriteit van kraakbeen en ontsteking van weefsel, rijk aan proteoglycanen. We diagnosticeerde relapsing polychondritis met een reversibele dementie, waarschijnlijk door een cerebrale vasculitis.

Reversible cognitive decline in a patient with relapsing polychondritis

Patient A, 58 years, is referred by the Rheumatologist to the Geriatrician concerning apathy. History reviews skipping arthritis, reddened and inflamed eyes, apathy and loss of interest. Physical examination revealed red tearing eyes, bradyphrenia and bradykinesia. Laboratory examination showed inflammation markers. The patient develops fever and bilateral reddened and inflamed ears. Diagnosis of relapsing polychondritis is made, the patient is treated with prednisone and the symptoms disappear.

Relapsing polychondritis is a chronical disease associated with inflammation and destruction of cartilaginous structures and proteoglycan rich structures.We diagnosed Relapsing Polychondritis with a reversible dementia,probably due to cerebral vasculitis.
