Tijdschrift praktijkondersteuning 2006;1(3):81-89.Stel dat de huisarts of praktijkondersteuner de zelfstandig wonende ouderen in zijn praktijk regelmatig thuis zou opzoeken. Zou dat eraan kunnen bijdragen dat zij lichamelijk minder snel achteruitgaan en dus pas later naar een verpleeghuis hoeven? Wij onderzochten dit door dertien gerandomiseerde gecontroleerde trials uit de periode 1980-2004 systematisch te vergelijken. In al deze onderzoeken ging het om een groep thuiswonende ouderen vanaf 65 jaar en keken de onderzoekers naar het dagelijks lichamelijk functioneren en het aantal opnames in een verpleeghuis.Uit de dertien onderzoeken blijkt dat preventieve huisbezoeken geen meetbare invloed hebben op het aantal verpleeghuisopnames. Hooguit patiënten die toch al niet veel risico lopen om te worden opgenomen zouden er baat bij kunnen hebben. Drie onderzoeken lieten zien dat huisbezoeken met een specifiek geriatrische invalshoek wel een positief effect kunnen hebben op het dagelijks lichamelijk functioneren. Dat effect is aangetoond bij twee groepen: vrouwen en ouderen die functioneel weinig achteruitgaan.
