Zusammenfassung
□ Hintergrund
Die immer komplexeren Möglichkeiten der modernen Medizin und der Wandel gesellschaftlicher Rahmenbedingungen (insbesondere die zunehmende Anerkennung der Bedeutung des Selbstbestimmungsrechts des Patienten) sind mit wachsenden Anforderungen an die ärztliche Gesprächskunst verbunden. Kommunikationsdefizite sind — von den Ärzten oft unbemerkt — jedoch weit verbreitet, insbesondere bei der Übermittlung schlechter Nachrichten. Schlüssel zu einer Verbesserung dieser Situation sind: Sorgfältige Vorbereitung der Rahmenbedingungen des Gesprächs; Vergewisserung über das Vorwissen, mit dem der Patient in das Gespräch geht; verständliche, dosierte Weitergabe der Information; Wahrnehmung der Patientenreaktion, insbesondere auf der emotionalen Ebene; und schließlich das Angebot weiterführender Gespräche.
Abstract
□ Background
In the more and more complex world of modern medicine, communication skills become increasingly important. Many physicians are however unaware of their difficulties in giving information in a clear and comprehensible way. This is especially true for breaking bad news. Keys for good communication consist in: careful preparation of the physical context of the interview; finding out how much the patient knows or suspects; giving the information in small amounts and a balanced way; responding to the patient’s feelings and finally making a contract for the future.
