Résumé
Cent dix patients porteurs d’un cancer gastrique épithélial ont subi une mucosectomie aspiration endoscopique. Nous présentons une évaluation de l’intérêt clinique de cette technique. Les patients ont été répartis en deux groupes. Groupe 1: indications thérapeutiques posées chez 52 patients porteurs de 56 lésions de carcinome gastrique d’un diamètre inférieur à 2,5 cm. Groupe 2: indications diagnostiques chez 58 patients porteurs de lésion tumorale «limite». Toutes les lésions ont pu être réséquées quelle que soit leur localisation. Dans le groupe 1, l’examen histologique a révélé que toutes les tumeurs à l’exception d’une seule (adénocarcinome peu différencié) étaient des adénocarcinomes bien ou modérément différenciés. La résection complète a été obtenue dans 78,6% des cas (44/56) de lésion du groupe 1, aucune récidive n’a été observée après une période de surveillance de 18 mois. Sept patients ayant subi une résection incomplète ont été ensuite soumis à un traitement chirurgical et trois lésions résiduelles ont été identifiées, exemptes de métastase lymphatique. Quatre patients sur les cinq restants du groupe 1 ont subi une résection incomplète qui a pu être contrôlée par un traitement endoscopique complémentaire et aucune lésion résiduelle n’a été retrouvée chez un autre patient. Dans le groupe 2, le diagnostic final d’adénocarcinome a été établi dans 14.1% des lésions (9/64). En raison du diamètre relativement large des lésions, une résection incomplète a été réalisée chez 7 d’entre eux. Une chirurgie complémentaire a été effectuée chez 4 patients suspectés de métastases ganglionnaires lymphatiques: néanmoins ils n’étaient porteurs ni de lésions résiduelles ni d’envahissement ganglionnaire. Aucune récidive n’a été observée chez les 3 patients restants. En conclusion, la mucosectomie aspiration endoscopique constitue une méthode utile au diagnostic et au traitement approprié de ces lésions.
Summary
One hundred and ten patients with a gastric epithelial tumor underwent endoscopic aspiration mucosectomy, and the clinical usefulness of this procedure was evaluated. The patients were divided into two groups. Group 1, with therapeutic indications was composed of 52 patients with 56 lesions of gastric carcinoma with a diameter <2.5cm. Group 2, with diagnostic indications was formed by 58 patients with 64 lesions of «borderline» tumors. All of the tumors could be resected regardless of their locations. In Group 1, the histologic evaluation revealed that all tumors but one (poorly differentiated adenocarcinoma) were well- to moderately differentiated adenocarcinomas. Complete resection was achieved in 78.6% (44/56) of the Group 1 lesions and no recurrence was found after a mean follow-up period of 18 months. Seven patients with incomplete resection underwent additional surgery, and three had residual lesions without lymph node metastasis. Four of the remaining 5 Group 1 patients with incomplete resection could be controlled by additional endoscopic treatment, and no remnant lesion was found in the other patient. In Group 2, the final diagnosis of adenocarcinoma was established in 14.1% (9/64) of the lesions. Because of the relatively large diameter of the lesions, incomplete resection occurred in seven of them. Although additional surgery was performed in 4 patients in whom lymph node metastasis was suspected, there were neither residual lesions nor lymph node involvement. No recurrence was found in the 3 remaining patients. In conclusion, endoscopic aspiration mucosectomy was found to be a useful modality for the diagnosis and treatment of gastric epithelial tumors.
