Résumé
Les effets cardio-vasculaires de l’Etomidate, injecté par voie intraveineuse à la dose de 0.2 mg/kg, ont été étudiés chez 14 patients chirurgicaux âgés de 49 à 90 ans.
L’induction du sommeil anesthésique survient dans les 10 secondes après l’injection et la durée moyenne de Fanesthésie est de six à huit minutes.
L’injection de l’Etomidate entraîne une baisse peu significative de la pression artérielle moyenne (8.5 pour cent), une négligeable accélération de la fréquence cardiaque (2.8 pour cent) et une diminution non significative de la pression moyenne dans l’artère pulmonaire (7 pour cent). Le débit cardiaque s’abaisse de façon modeste (7.6 pour cent) et le volume d’éjection systolique diminue d’environ 10 pour cent. Quant à la résistance vasculaire périphérique totale, elle montre une très discrète diminution de 3.8 pour cent.
Au vu de nos résultats et par rapport aux autres anesthésiques utilisés habituellement pour l’induction, l’Etomidate semble être le produit qui entraîne les plus faibles perturbations hémodynamiques. Toutefois, certains de nos patients ont présenté des effets secondaires gênants et en particulier des mouvements myocloni-ques et une douleur au niveau de Finjection endoveineuse.
Summary
The cardiovascular effects of intravenous Etomidate (0.2 mg/kg) were studied in 14 surgical patients with ages varying from 49 to 90.
The anaesthetic induction occurs within 10 seconds and the mean duration of anaesthesia is from 6 to 8 minutes.
I.V. Etomidate causes a slight lowering of the mean arterial pressure (8.5 per cent), a negligible increase of the heart rate (2.8 per cent) and an insignificant lowering of the mean pulmonary artery pressure (7 per cent). Cardiac output and stroke volume are respectively lowered by 7.6 per cent and 10 per cent. The peripheral vascular resistance is discreetly reduced by 3.8 per cent.
From these results, we believe that Etomidate seems to induce sleep with the lightest hemodynamic disturbances in comparison with other agents commonly used. However, some undesirable side effects appeared in some of our patients which can become somewhat annoying, for example myoclonic movements and pain at the point of injection.
