Résumé

945 cas de corps étrangers œsophagiens, traités dans le service ORL du CHUV entre 1963 et 1990, sont passés en revue. La prise en charge diagnostique et thérapeutique est décrite. La majorité des corps étrangers de I’œsophage se rencontrent chez le petit enfant et chez l’adulte entre cinquante et soixante ans. Chez l’enfant et chez l’adulte, la majorité des corps étrangers s’impacte au niveau du sphincter ésophagien supérieur, région techniquement peu accessible à 1’extraction endoscopique souple. Dans notre série, l’extraction a été endoscopique dans la majorité des cas (98,2 %) et la mortalité est de 0,2 %. L’efficacité et l’inocuité de 1’extraction endoscopique au tube rigide et en anesth’sie générate sont discutés.

Summary

945 cases of esophageal foreign bodies treated in the ENT service of the CHUV between 1963 and 1990 are reviewed. This study describes the diagnostic and treatment of these patients. The majority of cases of foreign bodies in the esophagus are found in young children and in adults aged between 50 and 60. These foreign bodies have a tendency to impact around the upper-esophageal sphincter, a region which, technically, is not very accessible for easy endoscopic flexible extraction. In our series extraction was carried out endoscopically in the majority of cases (98.2 %) and mortality stood at 0.2%. Thus, the efficiency and innocuity of endoscopic extraction with a rigid tube will be discussed in this text.

945 casos de cuerpos extraños esofágicos tratados en el Servicio ORL del CHUV entre 1963 y 1990, son revisados. El diagnóstico y la terapéutica son descritos. La mayoría de los cuerpos extraáos del esófago se encontraron en el niño y en el adulto entre cincuenta y sesenta años. En el niño y en el adulto, la mayoría de lo cuerpos extraños se impactó a nivel del esfínter esofógico superior, región tecnicamente poco accesible por extracción endoscópica con fibroscopio. En nuestra serie, la extracción ha sido endoscópica en la mayoría de los casos (98.2 %) y la mortalidad es de 0.2 %. La eficacia y la inocuidad de la extracción endoscópica con tubo rígido y con anestesia general son discutidas.
