Ergebnisse konservativ und operativ behandelter Knöchelbrüche

Zusammenfassung

Die operative Behandlung der Knöchelfrakturen hat sich in den letzten Jahren zu recht durchgesetzt. Anhand eines Kollektivs von 100 konservativ behandelten Malleolarfrakturen und eines zweiten von 105 operativ behandelten läßt sich insofern eine Aussage machen, als beide Gruppen nach den gleichen Kriterien kontrolliert und bewertet wurden. Maßstab hierfür waren die Einteilung der Frakturen in die drei Typen A, B und C nach DANIS bzw. WEBER und die Resultatsbewertung, wie sie WEBER 1972 vorgeschlagen und selbst an 150 Fällen durchgeführt hat. Die Untersuchung zeigt, daß gerade die Typen B und C der Operation zugeführt werden sollten. Voraussetzung für ein gutes Resultat ist einerseits die millimetergenaue anatomische Reposition des Außen- und Innenknöchels sowie die Wiederherstellung sämtlicher verletzter Bandverbindungen. Am Ergebnis selbst ist der traumatisch bedingte Knorpelschaden mitbeteiligt, auf den die Operation nur indirekten positiven Einfluß hat. Die erzielten Resultate zeigen große Übereinstimmung mit denen, die WEBER und WILLENEGGER publiziert haben.

Summary

In recent years, operative treatment of malleolus fractures has rightly established itself as the method of choice. A comperative study of 100 patients treated conservatively for malleolus fractures and 105 patients treated operatively was carried out under similar criteria. The fractures were classified under types A, B and C after DANIS or WEBER respectively and the results were analysed using the method recommended by WEBER in 1972 who analysed 150 cases. This analysis shows that particularly in types B and C operative treatment should be considered. To obtain good results, an exact anatomical reduction of the medial and lateral malleolus fractures as well as repair of all damaged ligaments is a prerequisite.

The results were influenced by damage to the cartilage which could only be indirectly positively influenced by the operation. Our results conform with those published by WEBER and WILLENEGGER.

Résumé

Le traitement par opération des fractures malléolaires s’est imposé ces dernières années à juste titre. Une étude comparative a été menée sur un premier groupe de 100 fractures traitées par conservation et d’un second groupe de 105 fractures traitées par opération. On a classé les fractures en 3 types A, B et C d’après DANIS, plus exactement WEBER et les résultats furent analysés d’après la méthode WEBER de 1972 (il a lui-même étudié 150 cas). L’examen montre qu’une opération s’imposait pour les types B et C. Condition pour un résultat favorable: d’une part, le replacement à un millimètre près du malléolaire externe et interne et d’autre part, la reconstitution de toutes les liaison touchées. Les résultats ont été entravés par l’endommagement du cartilage qui ne bénéficie qu’indirectement de l’opération. Les résultats obtenus sont conformes à ceux publiés par WEBER et WILLENEGGER.
