Zusammenfassung
Grundlagen: Sphinktererhaltende Verfahren sind als Verfahren der Wahl für Karzinome des oberen Rektumdrittels allgemein anerkannt, bei Tumoren des mittleren und unteren Rektumdrittels wird ihre Indikation noch kontrovers beurteilt.
Methodik: Unter den Sicherheitsabständen ist vor allem jener nach lateral von größter Bedeutung, seine Messung und Dokumentation ist allerdings noch nicht standardisiert. Für die Beurteilung des aboralen Sicherheitsabstandes ist die detaillierte Angabe der Meßbedingungen unerläßlich.
Ergebnisse: Bei kurablen Tumoren ist die nur mikroskopische kontinuierliche Ausbreitung und die lymphogene Metastasierung nach distal nur auf geringe Distanz zu beobachten. Im Mittelpunkt der Radikalitätsfrage steht die diskontinuierliche Ausbreitung im perirektalen Gewebe bzw. Mesorektum in Form sogenannter Satelliten. Diese finden sich auch distal des Tumors. Daher ist die komplette Entfernung des Mesorektums auch distal der Resektionslinie am muskulären Rektum erforderlich. Dies wird bei Stapleranastomosen auch durch technische Gründe nahe gelegt.
Schlußfolgerungen: Für weitere Studien zur Abklärung der Differentialindikation zwischen sphinktererhaltender radikaler Resektion und Rektumexstirpation ist die systematische Dokumentation definierter tumor- und therapieabhängiger Prognosefaktoren und ihre Bearbeitung mit multivariaten biometrischen Methoden erforderlich.
Summary
Background: Sphincterpreserving procedures are generally accepted for carcinomas of the upper third of the rectum; ther indication for tumors of the middle and lower third is still controversially discussed.
Methods: The lateral (circumferential) margin of clearance is of greatest importance, however its measurement methods and documentation are not yet standardized. For the assessment of distal margins of clearance detailed statements on measurement conditions are indispensable.
Results: In curable tumors, the histological continuous spread beyond the lower edge of tumor as well as the distal lymphatic spread are only limited. The focus of radicality lies in the discontinuous histological spread within the perirectal tissue or mesorectum as so-called satellites that can be observed also distally of the tumor. Therefore, the complete excision of the mesorectum also distally to the resection line on the muscular rectum is necessary. In stapled anastomosis, this is suggested also by technical reasons.
Conclusions: For further studies on differential indication between sphincter-saving radical resection and abdomino-perineal excision the systematic documentation of defined tumor- and treatment-associated prognostic factors and data analysis by multivariate biometric methods is essential.
