Zusammenfassung
Grundlagen: Sphinktererhaltende Resektionen haben heute einen festen Platz im chirurgischen Therapiekonzept des Karzinoms des mittleren und distalen Rektumdrittels.
Methodik: Die geschichtliche Entwicklung der sphinkter erhaltenden Eingriffe wird dargestellt.
Ergebnisse: In den beiden letzten Jahrzenten des 19. Jahrhunderts wurde die Exstirpation des Rektums durch einen sakralen Zugang erfolgreich durchgeführt. Wenige Jahre später wurde die abdomino-perinale Operation eingeführt und wurde in den nächsten 70 Jahren bevorzugt. Obwohl auch die Resektion in dieser Zeit entwickelt wurde, fand sie in Europa nur selten Anwendung. In den USA wurden sphintererhaltende Techniken während und nach dem 2. Weltkrieg entwickelt.
Erst systematische Untersuchungen zur Lymphdrainage des Rektumkarzinoms ins Periproktium bzw. zur Tumorzellausbreitung in der Rektumwand distal des Tumors bildeten die Grundlage zur Resektion ohne onkologischer Vorbehalte.
Schlußfolgerungen: Da die Ergebnisse der Exstirpation keine wesentlichen besseren und mit jenen der Resektion gut vergleichbar sind, sollte der sphinktererhaltenden Resektion der Vorzug gegeben werden, da diese Technik die normalen Verhältnisse nie vor der Erkrankung wiederherstellt.
Summary
Background: Sphincterpreserving resections are well established techniques in rectal carcinoma of the middle and distal parts.
Methods: The historical development of sphincterpreserving techniques are described.
Results: In the last two decades of the nineteenth century the exstirpation of the rectum by a sacral access was the first surgical treatment for a cancer situated in this area. But a few years later the abdomino-perineal access with a sigmoid colostomy got the preferred technique for the following seventy years. Though the resection was also developed, it was performed very seldom in Europe during the same period. But in USA the sphinctersaving techniques were newly introduced during and after the Second World War. After some successful investigations concerning the lymphdrainage of the rectal cancer in the periprocteal tissue and the extension of the cancercell infiltration in the rectal wall below the tumor border the resections could be performed easier without real oncological restrictions.
Conclusions: There the long-term results of rectal exstirpation are not different from those of resection/sphincterpreserving methods should be prefered. The benefit of these procedures are the restoration of the normal defecation and continence.
