Summary and Conclusions

Perforated diverticulitis with generalized peritonitis presents a clinical picture of acute abdomen not readily distinguishable from that caused by other perforations or vascular events; this alone precludes the safe use of conservative therapy of the acute stage followed by elective surgery as advocated by Byrne.

The method described offers safety that cannot be matched either by one-stage resection or by resection followed by establishment of a double-barrelled colostomy. Compared with the conventional three-stage resection it offers the advantages of wide resection of the diverticula-bearing tissue and elimination of a stage, which reduces mortality, morbidity and length of hospitalization.
