Zusammenfassung
204 Patienten mit konservativ behandeltem Hüftpfannenbruch wurden nachuntersucht. Das Intervall zwischen Unfall und Nachuntersuchung betrug 1,5 bis 16 Jahre, im Mittel 4,2 Jahre. Über die Häufigkeit der posttraumatischen Hüftkopfnekrose, Koxarthrose und paraartikulären Verkalkungen wird berichtet. Die Ermittlung des Gesamtergebnisses für jeden einzelnen Patienten erfolgte nach einem speziellen Auswertungsschema. Unverschobene Azetabulumfrakturen, dorsale Pfannenrandfrakturen mit kleinem Fragment und verschobene Pfannengrundfrakturen durch den vorderen Pfannenanteil zeigten nach konservativer Behandlung überwiegend gute Spätergebnisse. Die eindeutig schlechtesten Ergebnisse wurden bei dorsalen Luxationsfrakturen mit großem Randfragment und verschobenen Pfannengrundfrakturen durch biomechanisch wichtige Gelenkanteile (kranilaer und dorsaler Pfeiler) beobachtet.
Abstract
This paper describes a long-term follow-up study of 204 conservatively treated patients with acetabular fractures. The interval between accident and follow-up examination ranged from 1.5 to 16 years, with a mean interval of 4.2 years. The frequency of post-traumatic complications caused by avascular necrosis of the femoral head, coxarthrosis and periarticular calcification is reported.
Undisplaced acetabular fractures, posterior fracture dislocations with a minor acetabular rim fragment, and displaced fractures of the acetabular floor running through the anterior part of the acetabulum yielded predominantly good results when treated conservatively.
By far the poorest results were achieved in posterior fracture dislocations having a major rim fragment, and in displaced fractures of the acetabular floor which ran through biomechanically important parts of the acetabulum (cranial and posterior column).
