Simultaneous bilateral rupture of the achilles tendon following minimal trauma under steroid therapy—case report

Zusammenfassung

Anhand eines 69 jährigen Patienten wird ein Fall einer gleichzeitigen, beidseitigen Achillessehnenruptur dargestellt, welche durch Steroidtherapie induziert gewesen sein dürfte. Der Patient, der wegen rheumatischer Beschwerden unter täglicher Steroidtherapie steht, erleidet diese Verletzung bei einem Bagatelltrauma. Die Diagnostik, inklusive Magnetresonanz und Ultraschall, wird genau dargestellt. Es zeigt sich, daß die MRI eine ausgezeichnete Untersuchungsmethode für diese Verletzung ist. Die Operation, Bunnellsche Durchflechtungsnaht, wird in einer Sitzung an beiden Achillessehnen durchgeführt.

Abstract

The case of a bilateral simultaneous achilles tendon rupture under steroid therapy is reported in a 69 years old patients. The diagnosis includes clinical investigation, MRI, ultrasound and histological investigation. The steroid therapy was indicated because of a rheumatoid desease and pain in the shoulder. The cause of the influence of the steroid therapy as well as the literature supporting to this cases is discussed. Both achilles tendons were reconstructed in one operation.
