Summary
This study consists of 3 sections: a descriptive study of the intervertebral foramen by dissection, supplemented by millimetric sections made with the cinemicroabrasive apparatus (patented) on 12 specimens frozen with liquid nitrogen; a study of the attachments of the roots made during manipulations testing the resistance to avulsion of the rootlets, the roots, the dural sheath and the fibrous expansions at the periphery of the intervertebral foramen; and a study of the mobility of the roots in the foramina during movements effected within and outside the spine. We concluded that the classical description of an intervertebral foramen as occluded by a taut membrane like a drum-head is false as only perforated expansions attach the root to the foramen. Further, the points for attachment of the roots are the dura mater and the fibrous expansion passing from the foramen to the sheath of the spinal n. Finally, from the aspect of mobility, there are 2 distinct compartments and movements in one are not perceived in the other compartment if the attachments are unbroken. These attachments are a barrier interposed between the two compartments.
Résumé
Notre travail comporte 3 volets: premièrement, une étude descriptive du foramen intervertébral, par dissection; complétée par des coupes millimétriques faites au cinémicrotribeur (appareil breveté) sur 12 pièces congelées à l'azote liquide; deuxièmement, une étude des attaches des racines faites au cours de manipulations où sont testées les résistances à l'arrachement des radicelles, des racines, du fourreau duremérien, et des expansions fibreuses au pourtour du foramen intervertébral; et troisièmement, une étude de la mobilité des racines dans les trous de conjugaison lors de mouvements exercés en intra-rachidien et en extrarachidien. Nous avons conclu que la description classique du trou intervertébral obturé par une membrane tendue comme une peau de tambour (Forestier) est fausse car seules des expansions ajourées rattachent la racine au foramen. D'autre part, les points d'attache des racines sont la dure-mère et les expansions fibreuses allant du foramen à la gaine du n. rachidien. Enfin, du point de vue de la mobilité, il y a 2 compartiments distincts: intrarachidien et extra-rachidien. Les mouvements imposés dans un compartiment ne sont par perçus dans l'autre compartiment si les attaches ne sont pas rompues. Ces attaches sont une barrière interposée entre les 2 compartiments.
