Abstract
Cerebral ischemia was recorded in 1,9% of 1277 patients with myocardial infarction. In most cases ischemia involved the carotid artery system, usually causing a hemiparesis or hemiplegia. Patients were mostly elderly, and the ischemic episode worsened their prognosis. The pathogenesis was surely often of embolic origin but several facts suggest that other mechanisms were also involved. Anticoagulant therapy, at least in the form in which it was used in these patients, i.e. subcutaneous administration of calcium heparin 5000 I.U. b.i.d. for thrombophlebitis prophylaxis, does not seem to prevent these complications.

Su 1277 pazienti con infarto, si sono avuti fenomeni ischemici in ambito cerebrale nell' 1,9% dei casi. È risultato colpito quasi sempre il circolo carotideo, e ne è risultata in genere una emiparesi o una emiplegia. I pazienti colpiti erano per lo più anziani, e l'incidente ischemico ne ha aggravato la prognosi. Per quanto riguarda la patogenesi, essa sicuramente è spesso di natura embolica: una serie di considerazioni fanno ritenere operanti anche altri meccanismi. La terapia anticoagulante non sembra in grado di prevenire queste complicazioni, almeno nelle forme con cui è stata attuata in questi malati, e cioè con calcieparina sottocute per la prevenzione delle tromboflebiti.
