Pisa syndrome. Report of a case

Abstract

Very few cases of Pisa syndrome have been reported. The syndrome consists of dystonic symptoms, namely, tonic flexion of the trunk to one side and its slight rotation. It appears to be a side effect of prolonged antipsychotic therapy. We report on a case of Pisa syndrome in which withdrawal significantly improved the dystonic symptoms. However, a severe exacerbation of schizophrenic symptoms required the immediate resumption of neuroleptic therapy which was followed by the reappearance of dystonic symptoms. Associated anticholinergic medication led to only a slight improvement.

In letteratura sono stati riportati rari casi di sindrome di Pisa. La sindrome consiste in una distonia caratterizzata dalla flessione del tronco verso un lato e dalla sua rotazione e sembra essere causata da una prolungaterapia antipsicotica.

Gli Autori riportano un caso di sindrome di Pisa nel quale la sospensione dei farmaci determinò un considerevole miglioramento della distonia. Tuttavia una marcata accentuazione dei sintomi schizofrenici determinò l'immediato ripristino della terapia neurolettica che fu seguita dalla ricomparsa della distonia. Fu associata una terapia con farmaci anticolinergici che tuttavia portarono ad un miglioramento trascurabile dei sintomi.
