Abstract
Retrospective analyses of the electrocardiogram in 120 patients with chronic obstructive lung disease and cor pulmonale revealed cardiac arrhythmias in 30% of cases (36 patients). Supraventricular arrhythmias were about five times as frequent as those of ventricular origin. There was a remarkable frequency of paroxysmal atrial tachycardia with atrio-ventricular block not attributable to digitalis preparations being observed in 7 of 8 patients. In 10 of 36 patients the arrhythmias could be related to digitalis intoxication, in 3 other cases cardiac glycosides could not be definitely excluded as a cause of arrhythmias. 8 patients with severe chronic obstructive lung disease and bronchopulmonary infections with consecutive acute respiratory failure all showed multiple types of cardiac arrhythmias of predominantly supraventricular origin. However, no consistent relationship between the degree of arterial hypoxemia or hypercapnea and the appearance of arrhythmias could be demonstrated for the total of 120 patients with chronic obstructive lung disease.
Zusammenfassung
Rhythmusstörungen ließen sich bei 36 von insgesamt 120 stationär behandelten Pat. (30%) mit irreversibler, chronisch obstruktiver Lungenerkrankung und Cor pulmonale nachweisen. Davon zeigten 12 Pat. jeweils mehrere unterschiedliche Arrhythmietypen. Supraventrikuläre Rhythmusstörungen fanden sich etwa fünfmal häufiger als ventrikuläre Arrhythmien. Auffällig war die Häufigkeit von paroxysmalen Vorhoftachykardien mit atrioventrikulärem Block, die bei 7 Pat. unabhängig von den Einflüssen einer Digitalismedikation bestanden. Bei 10 der 36 Pat. waren die Rhythmusstörungen auf eine Digitalisintoxikation zu beziehen, in drei weiteren Fällen ließ sich eine Induktion der Arrhythmien durch Herzglykoside nicht sicher ausschließen. 8 Pat., die im Gefolge frischer bronchopulmonaler Infektionen mit konsekutiver, akuter respiratorischer Insuffizienz vorübergehend auf einer Intensivstation behandelt werden mußten, boten ausnahmslos multiple Formen vorwiegend tachykarder supraventrikulärer Arrhythmien. Dennoch ließ sich bei Zusammenfassung aller Patienten eine eindeutige Beziehung zwischen dem Auftreten von Rhythmusstörungen und dem jeweiligen Ausmaß der arteriellen Hypoxämie oder Hyperkapnie nicht sichern.
